    Abstract
Background. Systemic Lupus Erythematosus (SLE) is a chronic autoimmune disease that is heterogeneous and complex. Interleukin 6 (IL-6) is a cytokine that can increase in autoimmune diseases, this is supported by several studies related to the relationship between IL-6 and SLE. This study was conducted to determine the relationship between IL-6 levels and disease activity and manifestations of renal-nonrenal damage in SLE. Methods. The study was conducted retrospectively with a cross-sectional design on 67 people diagnosed with SLE based on the SLICC 2012/ACR EULAR 2019 criteria at least 6 months before being involved who were served at the Rheumatology Polyclinic of Haji Adam Malik General Hospital Medan during June to August 2023 who met the inclusion and exclusion criteria. Assessment of renal-non-renal damage manifestations using the SLICC Damage Index-SDI score and the degree of disease activity was measured using the SLEDAI- 2K. Results. The highest median IL-6 levels were seen in the group of patients with remission with a value of 87.32 pg/mL, while the lowest median IL-6 levels were seen in mild patients with a value of 46.05 pg/mL. There was no significant relationship between IL-6 levels and the degree of SLE disease activity (p=0.158). The median interleukin-6 value in the group of subjects with renal damage was 71.95 pg/ml while in the group of subjects with non-renal damage it was lower with a median of 53.05 pg/ml. Using the Mann Whitney test, it showed that there was a significant relationship between interleukin-6 levels and renal damage in SLE patients (p=0.045) Conclusion. There was no relationship between IL-6 levels and the degree of disease activity (p=0.158) but there was a significant relationship between IL-6 levels and renal damage (p=0.045).
