    Abstract
    Background: Hypoalbuminemia in critical illness is associated with organ failure and higher mortality. In adults, hypoalbuminemia is a significant indicator of mortality and morbidity. However, research on this matter, especially in children, is still few and limited. There is still controversy about the relationship between hypoalbuminemia and outcomes in children with critical illness. Objective: To determine the relationship between albumin levels and PELOD - 2 scores in children with critical illness in the PICU of Haji Adam Malik General Hospital, Medan. Methods: This study is a cross-sectional study of children who were admitted to the PICU of Haji Adam Malik General Hospital, Medan from January - September 2023. The PELOD - 2 score (<7 and ≥7) and albumin levels (≥ 3.5 g / dL and < 3.5 g / dL) were recorded in the first 24 hours of PICU care. Patients with malnutrition, malignancy, kidney failure and liver disorders were EXCLUDED in this study. Results: Forty research subjects were included in the study. In the group with albumin levels ≥3.5 g/dL, there were 12 children (80%) with PELOD - 2 scores <7 and 3 children (20%) with PELOD - 2 scores ≥7. In the group with albumin levels <3.5 g/dL, there were 8 children (32%) with PELOD - 2 scores <7 and 17 children (68%) with PELOD - 2 scores ≥7. Statistical analysis showed a significant relationship between albumin levels and PELOD - 2 scores (P = 0.003). Conclusion: Hypoalbuminemia in children with critical illness is associated with high PELOD - 2 scores.
