    Abstract
    Background: Ovarian cancer is a malignant disease whose management is quite difficult, because there is no prevention or early screening for diagnosis, this causes patients with ovarian cancer to more often present at an advanced stage and can reduce the prognosis and have a high recurrence rate. In recent years, vitamin D3 has been closely linked to its ability in antiangiogenesis, apoptosis and reducing metastases. This study aims to determine the role of vitamin D3 in the recurrence of ovarian cancer patients who have undergone optimal debulking and undergone complete chemotherapy. Methods: This study is an observational analytical study with a diagnostic test design for serum vitamin D3 as a protective factor in the recurrence of epithelial type ovarian cancer at Adam Malik Hospital, Medan using the case control method. This study involved 42 ovarian cancer patients who had total tumor debulking and underwent complete chemotherapy at least 1 year ago. Where subjects will be divided into two groups, patients with complete response (CR) and non-complete response (NCR). Data was taken from July to October 2024. Results: The average vitamin D3 levels in ovarian cancer based on response to chemotherapy (Complete Response/CR and Non-Complete Response/NCR) were 24.76 ng/mL and 16.67 ng/mL, respectively. It can be concluded that vitamin D3 levels were lower in the NCR group compared to the CR group. Most subjects in the CR group had vitamin D3 insufficiency (21–29 ng/mL), while most patients in the NCR group had vitamin D3 deficiency (< 20 ng/mL), and this was statistically significant (p=0.046). This study has included an analysis of vitamin D3 levels both based on vitamin D3 categorization (Deficiency, Insufficiency, and Adequate) as well as the overall average vitamin D3 levels in relation to chemotherapy response Conclusion: Vitamin D3 has been suggested to play a significant role in cancer recurrence prevention.
