    Abstract
Background: Chronic obstructive pulmonary disease is associated with acute exacerbations that can cause respiratory failure and is also a major cause of morbidity and death worldwide. Methods: Observational study with a retrospective cohort design using data from the medical records of patients diagnosed with chronic obstructive pulmonary disease with type II respiratory failure treated at H. Adam Malik General Hospital Medan for a period of 3 months from November 2021 to September 2022. The test used to analyze the relationship between the independent and dependent variables is the Chi-square test. To determine the independent variable that has the most dominant effect on the dependent variable, multiple logistic regression tests are used. Results: Patients who lived to the end of the study were 21 (50%), with a death rate of 21 of the 42 patients involved at the start of the study. ATH was 50% with a median survival of 14 days (95% CI: 7.178–20.822 days). The ATH of COPD patients with type II respiratory failure in patients who were not routinely treated was 16.7%, with a median survival of 4 days (95% CI: 42.962-5.039). The ATH of COPD patients with type II respiratory failure undergoing routine therapy is 75%. The log rank test showed that there was a significant difference in the median length of life between patients who did not regularly undergo therapy and patients who regularly underwent therapy (p 0.001). Conclusion: The survival rate in COPD patients who routinely use inhaler therapy is significantly higher than in patients who do not routinely use it.
