Hubungan Skoring Elektrokardiografi dengan Fraksi Ejeksi Ventrikel Kiri pada Pasien Gagal Jantung Kronik di RSUP H. Adam Malik Medan

Abstract

Background: Chronic heart failure is classified into three categories based on left ventricular ejection fraction, namely Heart failure with reduced ejection fraction (HFrEF), Heart failure with mid-range ejection fraction (HFmrEF), and Heart failure with preserved ejection fraction (HFpEF). This classification is useful for determining therapy and prognosis. ECG variable scoring can be used to predict left ventricular ejection fraction, thereby determining appropriate initial therapy based on the classification of chronic heart failure. Objective: This study aims to determine the correlation between electrocardiographic scoring and left ventricular ejection fraction in chronic heart failure patients at H. Adam Malik General Hospital, Medan. Method: This research is an observational study with a cross-sectional design with retrospective data collection from medical record sources. This research was conducted in June – December 2023 at H Adam Malik General Hospital, Medan. The sample for this study was all chronic heart failure patients from January - April 2023 at H Adam Malik Hospital Medan who met the inclusion and exclusion criteria. Selected subjects had electrocardiography and echocardiography data recorded, then data analysis was carried out. Results: The number of samples that met the inclusion criteria in this study was 60 people, and were divided into 3 groups. There were 31 people in the HFpEF group (52%), 12 people in the HFmrEF group, and 17 people in the HFrEF group (28%). There was a significant and in opposite direction correlation with moderate correlation strength for electrocardiographic scoring and left ventricular ejection fraction (r = -0,435, p = 0,001). Conclusion: There is a correlation between electrocardiographic scoring and left ventricular ejection fraction in chronic heart failure patients at H. Adam Malik General Hospital, Medan.
