    Abstract
    Introduction: Liver transplantation is a critical intervention for end stage liver diseases, yet challenges persist across pre--, peri--, and post transplant phases. This study aims to characterize potential deceased liver donors to inform transplantation practices and outcomes. Methods: A descriptive observational study with a case series design was conducted at H. Adam Malik Hospital, Medan, from September to December 2023. Medical records of Deceased Donor Liver Transplant (DDLT) candidates were analyzed for demographic and clinical characteristics using SPSS software. Results: Of the 100 DDLT candidates analyzed, the majority were male (66%) with a mean age of 60.73 years. Blood type O was most common (40%), and cardiovascular disease was the leading etiology (38%). All donors were viable, with signs of brainstem death present in all cases. Discussion: Liver transplantation has evolved over decades, yet challenges persist, including donor shortages and disparities in recipient access. Age related factors and donor recipient matching play significant roles in transplant outcomes. Education level and blood type influence organ allocation and waiting times. Optimizing organ utilization and allocation remains crucial for improving transplant practices and outcomes.
