Background : Stroke is a global health problem today that can cause death or disability. The majority of strokes are ischemic strokes and occur in the anterior circulation. Previous study shows that several parameters from transcranial Doppler examinations in the form of Pulsatility Index (PI) and Asymmetry Index Middle Cerebral Artery (MCA) as well as blood biomarker parameters in the form of Atherogenic Index Plasma (AIP) have a correlation with the NIHSS (National Institute of Health Stroke Scale) score and can predicting ischemic stroke outcome. Research on this parameter is still limited in Indonesia. Objectives : to determine the correlation between AIP, PI, and MCA Asymmetry Index using transcranial Doppler with the NIHSS score. Methods :. This research was correlative analytical with a cross-sectional design. The research sample consisted of 38 acute ischemic stroke patients at Haji Adam Malik Hospital Medan in November 2023 – August 2024. Researchers assessed AIP based on lipid profile examination while PI and MCA Asymmetry Index from Trans Cranial Doppler examination then researchers assessed the NIHSS score on the seventh day of onset stroke. Data analysis used the Spearman correlation test. Correlation tests were carried out between AIP, PI and Asymmetry Index with the NIHSS score. Results : This research was conducted of the 38 subject, the majority were male (76.3%), median age was 56 years (22-65). Mean AIP 0.18 (±0.2), mean MCA PI 1.31 (±0.20), median asymmetry index 22.86 (3.16-62.21), median mean flow velocity 39,65 (19,60-84,30) cm/s and median NIHSS score 6 (3-12). A significant moderate positive correlation existed between the asymmetry index and the NIHSS score (r=0.491; p=0.002). There was no significant correlation between AIP and PI with NIHSS scores. Conclusion : There is a positive correlation between the asymmetry index and the NIHSS score.en_US
