    Abstract
Sexual orientation is a multidimensional phenomenon involving a person's sexual interests, sexual behavior, and sexual identity. Types of sexual orientation are heterosexual, homosexual and bisexual. Gay is often defined as a man who likes other men or has a sexual attraction to men and has a homosexual sexual orientation. This research aims to determine the causes of gay sexual orientation and its impact on the reproductive health of late adolescents in Medan City. The subjects in this research were young people aged 18–24 years who had a gay sexual orientation in the city of Medan. Data collection techniques use observation and in-depth interviews. The results obtained were the discovery of the factors that caused the informant to become gay, namely because of bullying, the habit of watching pornographic films, joining a community of same-sex fans, peers, family, and feelings of hurt towards the opposite sex. The main factor causing gay sexual orientation in informants is the environment. The environment plays an important role in forming an individual's mindset and mentality towards becoming gay. All information has the habit of having anal or oral sexual intercourse. The reproductive health impacts felt by informants include sexually transmitted infections (STIs) and gender discrimination resulting in feelings of discomfort in everyday life.
