Correlation between Lipid Profiles and Atherogenic Index with the Severity of Diabetic Foot Based on the Wagner-Meggitt Criteria at Adam Malik Hospital

Abstract: Diabetic foot is a serious complication in patients with diabetes mellitus that can lead to ulcers, infections, and amputations. Identifying risk factors that influence the severity of diabetic foot is crucial for effective prevention and management. Lipid profiles and atherogenic indices have been widely identified as potential risk factors for atherosclerosis, cardiovascular disease, and peripheral arterial disease; however, their relationship with diabetic foot severity is not yet fully understood. This study aims to investigate the correlation between lipid profiles and atherogenic indices with the degree of severity of diabetic foot based on the Wagner-Meggitt criteria at RS. Adam Malik.
Methods: Analytical observational study with cross-sectional design was conducted at RS. Adam Malik in 70 diabetic foot patients. Data were obtained from secondary medical records, including lipid profile data such as total cholesterol (TC), LDL, HDL, and triglycerides (TG). The atherogenic index was calculated based on the ratio of total cholesterol to HDL (TC/HDL), triglycerides to HDL (TG/HDL), and LDL to HDL (LDL/HDL). The severity of diabetic foot was assessed using the Wagner-Meggitt classification. Data analysis was performed using statistical tests with Chi-square and Spearman's rho analysis.
Results: From a total of 70 samples obtained, the majority of patients were aged 51-60 years (41.4%), male (60%) and a normal BMI category (57.1%). A significant relationship was found between the degree of severity of diabetic foot and TG levels (p=0.0002, r=0.358) and the TC/HDL ratio (p=0.02, r=-0.277). In contrast, TC levels (p=0.533), HDL (p=0.254), LDL (p=0.533), TG/HDL ratio (p=0.158), and LDL/HDL ratio (p=0.92) did not show a significant relationship with the degree of severity of diabetic foot.
Conclusion: Triglycerides and the total cholesterol/HDL ratio are significantly related to the degree of severity of diabetic foot based on the Wagner- Meggit criteria.
