dc.description.abstractIn this modern time companies have to survive in increasingly competitive market, one of the many ways to do that is to improve quality of the product. Any error or mistake in production will result in defective products or product variations, bring losses to the company. PT Berdikari Plastik is a company that produces recycled plastic pellets and plastic bags. This research was conducted to determine the value of financial loss due to product variations and defects and the factors that contribute to it. Loss calculations using Taguchi's Quality Loss Function are carried out to provide information about production conditions. This research found that production of plastic pellet losses of IDR 1,881,685.74 per month and IDR 12,075,885.99 for the production of plastic bags. Questionnare was filled out by the factories operators and analyzed with Failure Mode and Effect Analysis. It is found that the most prominent risk factor of in the production of plastic pellets is machine factor and human factor for plastic bag production. Recommended actions that would be proposed are to make checklist on setting up the blowing machine and to do maintenance periodically for the cutting machine.en_US
