    The Impact of Home Care Pharmacy on Knowledge, Adherence, Therapy Success, and Quality of Life of Diabetes Mellitus Patients with Referral Program

    Abstract
    Diabetes Mellitus (DM) until now remains a major public health issue worldwide and causes many deaths, including in Indonesia, thus requiring better disease management. The Referral Program (RP) is one of the government programs that provides healthcare services to DM patients; however, the medication redemption conducted once a month results in insufficient monitoring of patient treatment. Home Pharmacy Care (HPC) is a form of pharmaceutical service through education, counseling, and medication therapy monitoring that can help manage DM disease in RP participants. This study aims to analyze the impact of HPC on the level of knowledge, adherence, therapy success, and quality of life of DM patients participating in RP. The research was conducted at Sering Primary Health Center Medan for three months using a quasi-experimental design with a one-group pretest- posttest. The sample selection used the total sampling method, involving 60 active DM patients participating in RP. The research instruments included the Diabetes Knowledge Questionnaire-27 (DKQ-27), the Medication Adherence Report Scale (MARS), the Diabetes Quality of Life (DQoL) questionnaire, and the fasting blood glucose examination sheet. The research results show that there are significant differences in the levels of knowledge, adherence, therapy success, and quality of life before (pretest) and after HPC (posttest). Based on the Wilcoxon test results, the average pretest and posttest scores of the respondents were as follows: knowledge: 11.17±4.24 and 22.65±2.97 (p<0.001), adherence: 19.40±2.24 and 24.82±0.43 (p<0.001), therapy success: 288.78±67.80 and 194.67±46.54 (p<0.001), and quality of life: 35.52±1.65 and 45.32±3.79 (p<0.001). The conclusion of this study is that HPC improve knowledge, adherence, therapy success, and quality of life for DM patients participating in RP at Sering Primary Health Center
