    Abstract
    Introduction: Ischemic stroke can trigger an inflammatory process. This inflammatory state can lead to infarct expansion and thus induce hypercatabolism leading to worsening clinical outcomes. Premorbid malnutrition has been shown to have an adverse effect on the prognosis of stroke patients. It is associated with increased complications, mortality, length of hospitalization, and poor neurological outcomes in acute stroke patients. Therefore, it is important to evaluate nutritional status appropriately and provide appropriate nutritional supplements. Objective: To determine the relationship between nutritional status based on the Geriatric Nutritional Risk Index score and changes in severity in acute ischemic stroke patients. Methods: This study uses a cohort design in patients diagnosed with acute ischemic stroke who are undergoing treatment at H. Adam Malik Hospital Medan. Nutritional status will be measured using the Geriatric Nutritional Risk Index (GNRI) score and stroke severity will be measured using the National Institutes of Health Stroke Scale (NIHSS) score. Measurement of NIHSS score will be done twice, namely at the time of admission and the seventh day after admission. Results: The incidence of malnutrition based on GNRI scores in the elderly who experienced ischemic stroke was 74.6%. The results of statistical analysis showed significant results between nutritional status based on GNRI scores on changes in severity in acute ischemic stroke patients (r=0.655, P<0.001). Conclusion: There is a significant relationship between nutritional status based on GNRI score and changes in severity in acute ischemic stroke patients
