    Analysis of Category I Cesarean Section Response Time on Maternal- Neonatal Outcomes at Adam Malik Hospital Medan

    Abstract
    Background : Maternal mortality rates are high, around 287,000 women dying during and after pregnancy and childbirth. The global under-five infant mortality rate was 37 deaths per 1,000 livebirth. category I cesarean section is a cesarean section where there is a direct threat to the life of the mother or fetus, from the time the operation is decided until the start of the surgical incision in the operating room is ≤ 30 min. Aims : To analyze the decision-to-incision interval response time <30 min in category 1 emergency cesarean sections at Adam Malik Hospital Medan. Method : This study was conducted using a retrospective cohort method to assess the response time for category 1 emergency cesarean sections and the factors affecting response time. A total 44 samples undergone category 1 emergency caesarean section were included. Statistical analysis using SPSS version 22 (IBM Corporate). Bivariate and multivariate logistic regresion with a 95% confidence interval to evaluate factors that affecting response time. Result : Only 43.2 % had decision to incision interval < 30 min. Significant association affecting response time delay were found for transfer time (COR = 16.89; p < 0.001) and anesthesia time (COR = 32.00; p < 0.001). On maternal and neonatal outcomes, no significant association was found with emergency caesarean section category 1 response time. Conclusion : The transfer time and anesthesia time were factors that affecting the response time delay. The average decision to incision interval was longer than recommended time but did not affect maternal-neonatal outcomes.
