dc.description.abstractHuman error is an error resulting from the performance of activities that should not be carried out so that it can result in chaos or disruption to operations or cause damage to equipment and supplies. in the last three years a number of PT. Simpang Kanan Lestarindo experienced accidents and losses during the production process caused by worker negligence and human error. The aim of this research is to find out what factors can cause human error in PT employees. Simpang Kanan Lestarindo and Providing suggestions for improvements that will be made to reduce human error. The results of the HEART method obtained are based on the highest % HEP calculation, namely at the press station at 23%. And the results of the SHERPA method are at the sterilizer station with the highest score of 16, ordinal high (H) analysis.
