    Abstract
    Background : Intracranial meningioma was one of the most common primary intracranial tumors, accounting for 10-15% of all brain neoplasms worldwide. Magnetic resonance imaging (MRI) was the modality of choice for diagnosing meningioma. The apparent diffusion coefficient (ADC) was an MRI sequence that quantitatively measured values calculated from the region of interest (ROI) to determine how freely water molecules could diffuse. ADC values varied depending on tumor consistency. Objective : This study aimed to determine the correlation between ADC values in MRI examinations and the consistency of intracranial meningioma based on histopathological examination at Adam Malik Hospital. Methods : This study was an analytical observational study with a diagnostic test design using head MRI with contrast results at Adam Malik Hospital from August 2022 to August 2024. Results : Among 68 meningioma patients at Adam Malik Hospital, 50 (73.5%) were male, and 18 (26.5%) were female. The most common age ranges were 35–44 years and 45–54 years, each accounting for 21 patients (30.9%). The mean ADC value for soft-consistency meningiomas was ±1.349 × 10⁻³ mm²/s, for medium-consistency meningiomas was ±0.873 × 10⁻³ mm²/s, and for hard-consistency meningiomas was ±0.687 × 10⁻³ mm²/s. The ADC cut-off value for differentiating meningioma consistency was 0.98167 × 10⁻³ mm²/s. The sensitivity, specificity, positive predictive value, and negative predictive value of ADC in determining meningioma consistency were 87%, 88%, 84%, and 85%, respectively. Statistical analysis showed a very strong correlation between ADC values and meningioma consistency in patients who underwent head MRI, with r = -0.841. Conclusion : There was a very strong negative correlation between ADC values in contrast-enhanced head MRI and meningioma consistency based on histopathology at Adam Malik Hospital.
