Background: Ascites is the pathological accumulation of fluid in the peritoneal cavity and is the most common complication of cirrhosis, occurring in approximately 50% of patients with decompensated cirrhosis within 10 years. Diagnostic paracentesis with proper ascitic fluid analysis must be performed on all hospitalized patients with ascites to exclude non-cirrhotic causes. This study aimed to analyze the differences in ascitic fluid profile between cirrhotic and non- cirrhotic ascites patients at Adam Malik General Hospital. Methods: This was an observational analytic study with a case-control design, utilizing secondary data from medical records of patients diagnosed with cirrhotic and non-cirrhotic ascites who underwent paracentesis and ascitic fluid analysis during 2023. Results: Statistically significant correlations (p < 0.001) were found between ascitic fluid parameters: appearance (color), glucose, total protein, total LDH, and white blood cell count (WBC). Fluid color showed a negative correlation with total protein (r = -0.474) and LDH (r = -0.576), and a positive correlation with WBC (r = 0.355). Glucose had a moderate positive correlation with total protein (r = 0.352) and LDH (r = 0.429), and a weak negative correlation with WBC (r = - 0.206). Total protein showed a strong positive correlation with LDH (r = 0.661) and a moderate negative correlation with WBC (r = -0.317). LDH had a moderate- to-strong negative correlation with WBC (r = -0.462). Conclusion: There is a significant correlation between biochemical and cellular components of ascitic fluid. Increased inflammatory cells (WBC) are often accompanied by decreased glucose and protein levels, and darker or cloudier fluid appearance, indicating possible infection or malignancy in ascitic fluid.en_US
