Auditory hallucinations are a common perceptual disturbance in individuals with mental illness and may trigger aggression, social isolation, and psychosocial dysfunction. Appropriate nursing interventions are needed to help clients manage these symptoms adaptively. One approach used is Acceptance and Commitment Therapy (ACT), which assists clients in accepting hallucinations without emotional resistance. The intervention was carried out in four sessions at Prof. Dr. Muhammad Ildrem Mental Hospital, Medan, focusing on experiential acceptance, cognitive defusion, values clarification, and commitment to adaptive behavior. Techniques employed included mindfulness, values identification, commitment training, and education on relapse prevention. The results demonstrated a reduction in the frequency of hallucinations, improved emotional regulation, and increased client commitment to coping strategies such as challenging the voices, praying, and participating in social activities independently. In conclusion, ACT is effective as a non-pharmacological intervention for reducing auditory hallucination symptoms and fostering adaptive behavior in individuals with mental illness.
