Efektivitas Pengarahan Kepala Ruangan dalam Pelaksanaan Discharge Planning di Lingkungan Ruang Rawat Inap Anak RS. Prof. Dr. Chairuddin Panusunan Lubis

Abstract

Discharge planning is a crucial process in ensuring continuity of healthcare services and patient safety after discharge from the hospital. However, in practice, its implementation in the field remains suboptimal due to noncompliance with the established Standard Operating Procedures (SOPs). Therefore, optimal guidance through the directing function of the head nurse is necessary. This senior practicum aimed to evaluate the effectiveness of the head nurse’s guidance in implementing the discharge planning SOP in the pediatric inpatient ward of Prof. Dr. Chairuddin Panusunan Lubis Hospital. The method used was a case study approach based on Kurt Lewin’s change theory and a SWOT analysis to formulate intervention strategies. The implementation was carried out over two weeks through direct observation, discussions with the head nurse, the development of a discharge planning SOP, and a trial of structured guidance using a checklist instrument. The results showed improvements in the organization and timeliness of discharge planning, increased compliance with completing discharge planning forms, and enhanced responsibility and readiness among nurses. The structured guidance also fostered the development of a more professional and reflective work culture. These findings indicate that effective guidance provided by the head nurse not only improves adherence to the discharge planning SOP but also strengthens team communication and patient safety. It is recommended that the head nurse adopt the discharge planning SOP as part of ongoing efforts to enhance the quality of nursing services.
