dc.description.abstractSensory perception disorders in the form of auditory hallucinations are one of the positive symptoms found in patients with chronic mental disorders. These positive symptoms can affect the individual's social, emotional, and cognitive functions. If hallucinations cannot be controlled, it will have an impact on the risk of violent behavior directed at oneself, others and the environment. Clients need immediate intervention to help them control these symptoms, namely by providing nonpharmacological therapy such as Acceptance and Commitment Therapy (ACT). This report aims to implement the management and nursing care of clients with sensory perception disorders: auditory hallucinations through the application of ACT in Medan Denai District. The instruments used in this study are in the format of lecture, discussion, demonstration and redemonstration methods. which includes six core processes: acceptance, cognitive defusion, contact with the present, self as context, life values, and commitment to action. The results of this nursing care provision showed that the client showed a decrease in the frequency of auditory hallucinations, an increase in the ability to divert attention from hallucinatory stimuli, and an increase in awareness and commitment to carry out rebuke exercises, medication compliance and relapse prevention after attending three ACT therapy sessions. ACT therapy has been shown to have a positive impact on the client's knowledge and skills in recognizing and controlling hallucination symptoms. This therapy can be used in community mental health practice settings.en_US
