    Abstract
    Stroke is a condition that occurs when the blood supply to the brain is disrupted or when there is bleeding in the brain tissue, resulting in loss of brain function. The incidence of pressure ulcers among stroke patients is notably high, particularly in patients develop pressure ulcers during hospitalization. This nursing care intervention aims to reduce the risk level of pressure ulcers in stroke patients through scheduled right and left lateral positioning, monitoring of skin integrity, and the application of olive oil. The method involved a nursing care approach for three stroke patients. The intervention was conducted over a two-week period from june 10 tu june 21 2025, and included assessment, nursing diagnosis, planning, implementation, and evaluation. The implementation was carried out for three consevutive days, involving repositioning the patient to the right and left sides every two hours, observing skin integrity, and applying olive oil twice daily (morning and evening) for 20 minutes. Patient and family education on pressure ulcer prevention, skin assessment, and proper application of olive oil was also provided. The results showed a decrease of 1-2 points in pressure ulcer risk scores in all three patients from day one to day three. These findings are consistent with previous studies indicating that two-hourly repositioning, skin observation, and olive oil application can effectively prevent and reduce the risk of pressure ulcers in stroke patients. This practicum demonstrates a simple, non-pharmacological, and cost-effective nursing intervention, which may also be optimized by adjusting repositioning frequency to every one or three hours for comparative evaluation of effectiveness.
