    Abstract
Background: Health is a fundamental human right and a crucial element of well-being that must be realized, and health efforts are necessary to ensure public health. The Primary Health Care Center (Puskesmas) is a basic health service facility that provides comprehensive, integrated, and continuous efforts for promotional, preventive, curative, and rehabilitative health care. Pharmaceutical Services at the Puskesmas are very important in improving the quality of health services. These services are required to shift from the old paradigm which is product-oriented (drug oriented) to a new paradigm that is patient-oriented (patient oriented or pharmaceutical care). Objective: The main objective of this PKPA is to prepare prospective pharmacists to understand the role, function, and responsibility of a Pharmacist in the Puskesmas in supporting health services. Summary: The Pharmacist Professional Practice Internship (PKPA) was carried out at UPT Puskesmas Amplas, Medan, from November 01 to November 28, 2024. The activities covered two main aspects. First, the management of pharmaceutical supplies, medical devices, and consumable medical supplies (BMHP), which includes needs planning, procurement requests, receiving, storage, distribution, control, recording, reporting, archiving, as well as monitoring and evaluation of management. Second, clinical pharmacy services, which include prescription review, drug dispensing and providing drug information, Drug Information Services (DIS), Counseling, Monitoring and Reporting of Adverse Drug Reactions (ADR), Drug Therapy Monitoring, and Drug Use Evaluation. Conclusion: The implementation of the PKPA at UPT Puskesmas Amplas has provided prospective pharmacists with an in-depth understanding and direct experience regarding the management of Pharmaceutical Supplies, Medical Devices, and BMHP, as well as the implementation of clinical pharmacy services in accordance with applicable standards. This comprehensive activity successfully equipped the prospective pharmacist to be capable of carrying out pharmaceutical practice in the Puskesmas professionally, in accordance with the prevailing laws, regulations, and professional principles.
