| dc.description.abstract | Background: Hospitals are healthcare facilities that provide comprehensive services
including promotive, preventive, curative, and rehabilitative care. The hospital pharmacy
plays a crucial role in ensuring the availability of pharmaceutical preparations, medical
devices, and clinical pharmacy services while prioritizing patient safety. The paradigm shift
from drug oriented to patient oriented demands pharmacists not only act as medication
providers but also as integral members of the healthcare team in optimizing patient therapy.
Therefore, the Pharmacist Professional Work Practice (PKPA) program serves as a vital
platform to equip prospective pharmacists with real-world experience in the
implementation of pharmaceutical care based on current regulations.
Objective: The Pharmacist Professional Work Practice (PKPA) at Murni Teguh Memorial
Hospital Medan aims to enhance the understanding of prospective pharmacists regarding
the roles, functions, and responsibilities of pharmacists in pharmaceutical supply
management and clinical pharmacy services, in accordance with the Indonesian Ministry
of Health Regulation No. 72 of 2016, while strengthening professional readiness in hospital
practice.
Summary: The PKPA was conducted from February 3 to March 31, 2025, at the Pharmacy
Department of Murni Teguh Memorial Hospital. The activities included pharmaceutical
supply chain management using a one-gate system (selection, planning, procurement,
receipt, storage, and distribution), as well as participation in clinical pharmacy services
such as medication history review, medication reconciliation, patient counseling, ward
rounds, therapeutic drug monitoring (TDM), and adverse drug reaction (ADR) monitoring.
Overall, the pharmacy services were in accordance with current regulations, although
Therapeutic Drug Monitoring (TDM/PKOD) has not yet been fully implemented.
Conclusion: The PKPA program at Murni Teguh Memorial Hospital provided
comprehensive exposure to the role of pharmacists in supporting quality healthcare services
and patient safety. This experience enhanced the professional competencies of prospective
pharmacists in both pharmaceutical logistics and clinical pharmacy services, preparing
them to enter the professional healthcare setting with confidence. | en_US |
