Background: Death due to asphyxia is crucial forensic case with a complex classification of the manner of death. The epidemiological profile of asphyxial deaths caries significantly across regions and requires in-depth analysis for legal and prevention purposes. Objective: This study aims to provide a descriptive profile of deaths cases due to asphyxia based on the characteristic of age, sex. mechanism of ashpyxia, manner of death, and specific asphyxial signs. Methods: This descriptive study analyzed 25 asphyxial death cases recorded through Visum et Repertum at Bhayangkara Hospital Medan during January 2022–January 2023. Data were analyzed univariately with descriptice interpretation. Results: The dominant manner of death was homicide (60%), a finding that strongly contrasts with many global reports often dominated by suicide. The most common mechanism were strangulation (36%) and drowning (32%). The gender distribution was relatively balanced between males (52%) and females (48%). The most frequent asphyxial signs were cyanosis (76%) and fine foam (56%). Conclusion: The dominance of homicide as manner of death suggests that asphyxia case sample in this study area exhibits a highly criminogenic profile. The pattern of asphyxial mechanisms reflects a duak risk for the study population which are high intentional violence and enviromental hazards.en_US
