    Analisis Mikroorganisme pada Akseptor Alat Kontrasepsi dalam Rahim (AKDR) di RSUD DR. Pirngadi Medan

    Abstract
    Latar Belakang : Tahun 2015, 14% wanita di seluruh dunia menggunakan AKDR. Penggunaan AKDR terkait peningkatan risiko infeksi karena flora normal cervico-vaginal berkolonisasi di perangkat AKDR dan membentuk biofilm yang terdiri dari lapisan sel induk dan bakteri/jamur. Tujuan : Penelitian ini memberikan informasi tentang jenis mikrorganisme vagina pada akseptor AKDR. Metode : Penelitian deskriptif dengan rancangan case series pada 40 akseptor AKDR di RSUD Dr. Pirngadi Medan mulai dari Januari hingga jumlah sampel terpenuhi. Dilakukan anamnesis dan pencatatan antopometri. Kemudian pengukuran keasaman vagina dengan indikator pHstrips dan pemeriksaan mikrobiologis swab vagina. Data dikumpulkan kemudian dianalisis secara statistik univariat. Hasil : Distribusi frekuensi paling banyak dijumpai pada kelompok usia 30 - 40 tahun dengan 21 orang (52,5 %), multipara 21 orang (52,5 %) dan obesitas 17 orang (42,5 %). Lama penggunaan AKDR yang terbanyak selama 2 – 4 tahun dengan 19 orang (47,5 %) dan nilai pH vagina ≤ 5 dengan 29 orang (72,5 %). Mikroorganisme Aerob yang terbanyak adalah Staphylococcus aureus (18%) dengan sensitif terhadap Tigecycline (11,4%) , Escherichia coli (13,5%) dengan sensitif terhadap Sulfametaxazole/Trimetoprime (8,3%), Gardnerella vaginalis (11,2%) dengan sensitif terhadap Ciprofloxacin (18,3%) . Bakteri anaerob terbanyak adalah Bacteroides sp (15,0%), Porphyromonas sp (5,0%) dan Prevotella sp (2,5%). Pertumbuhan jamur yang terbanyak adalah Candida albicans (37,5%) dengan 10,14 sensitif terhadap Flucytocin. Didapatkan keluhan keputihan sebanyak 29 orang (72,5%). Kesimpulan : Dari pemeriksaan swab vagina, mikroorganisme yang paling banyak dijumpai pada akseptor AKDR adalah Staphylococcus aureus (Aerob), Bacteroides sp. (Anaerob), dan Candida albicans (Jamur) dengan mayoritas pH vagina 5 dan keluhan terbanyak adalah keputihan .
     
    Background: In 2015, 14% of women around the world used an IUD. IUD use is associated with an increased risk of infection because the cervico-vaginal normal flora colonizes the IUD device and forms biofilms consisting of layers of stem cells and bacteria / fungi. Objective: This study provides information about the types of vaginal microorganisms in IUD acceptors. Method: A descriptive study with a case series design on 40 IUD acceptors at the DR. Pirngadi Hospital Medan. Research starts from January until the number of samples is fulfilled. History and anthropometric recordings are performed. Then measurement of vaginal acidity with pH strips indicator and microbiological examination of vaginal swabs. Data collected then analyzed statistically univariate. Results: Frequency distribution was most often found in the age group 30-40 years with 21 people (52.5%), multipara 21 people (52.5%) and obesity 17 people (42.5%). The duration of IUD use was the most for 2-4 years with 19 people (47.5%) and vaginal pH values ≤ 5 with 29 people (72.5%). Most aerobic microorganisms were Staphylococcus aureus (18,0%) with results sensitive to Tigecycline (11,4%) , Escherichia coli (13.5%) with results sensitive to Sulfametaxazole/Trimetoprime (8,3%) , Gardnerella vaginalis (11,2%) with the results sensitive to Ciprofloxacin (18,3%). The most anaerobic bacteria were Bacteroides sp (15.0%), Porphyromonas sp (5.0%) and Prevotella sp (2.5%). The most fungal growth was Candida albicans (37.5%) with 10.14 sensitive to Flucytocin. Obtained vaginal discharge complaints with a total of 29 people (72.5%). Conclusion: From vaginal swab examination, the most common microorganisms found in IUD acceptors are Staphylococcus aureus (Aerob), Bacteroides sp. (Anaerobic), and Candida albicans (Mushroom) with the majority of vaginal pH 5 and most complaints are vaginal discharge.
