    Pengaruh Polimorfisme Gen Reseptor Vitamin D BSMI dan Kadar Vitamin D Plasma Darah Terhadap Resiko Terjadinya Psoriasis

    Abstract
    Psoriasis merupakan penyakit autoimun yang bersifat kronik dengan prevalensi 2-3% dari keseluruhan populasi dunia. Gen RVD BsmI merupakan salah satu gen yang berada di intron 8 dan belum banyak diteliti pada kelompok psoriasis di Indonesia. Penelitian ini bertujuan untuk mengetahui pengaruh polimorfisme gen RVD BsmI dan kadar plasma vitamin D terhadap risiko terjadinya psoriasis. Metode penelitian ini adalah case control terhadap 44 subyek kelompok psoriasis dan 44 subyek kelompok tidak psoriasis dengan usia >18 tahun. Dilakukan PCR-RFLP terhadap hasil isolasi DNA dan ELISA pada plasma darah. Analisis data dilakukan dengan uji Chi Square. Hasil analisis didapat jenis kelamin perempuan paling banyak pada kedua kelompok (kasus:56,8% ; kontrol:72,7%) dengan usia rerata 47 tahun pada kelompok kasus dan 24 tahun pada kelompok kontrol. Varian GG (bb) lebih banyak terdapat dikedua kelompok (kasus:77,3%;kontrol:81,8%) dibandingkan varian GA (Bb). Pada penelitian ini tidak terdapat varian AA (BB). Rerata kadar vitamin D pada kelompok kasus 25,92 ng/ml dan kelompok kontrol 12,96 ng/ml. Kadar plasma vitamin D pada kelompok kasus (Defisiensi: 36,4%; Insufisiensi: 31,8% ; Sufisiensi: 31,8%) sedangkan pada kelompok kontrol (Defisiensi:81,8%; Insufisiensi: 15,9%; Defisiensi: 2,3%). Dapat disimpulkan bahwa terdapat hubungan yang bermakna antara kadar plasma vitamin D dengan kejadian psoriasis (p<0,05), sebaliknya tidak terdapat hubungan yang bermakna antara varian GG (bb) dan GA (Bb) dengan kejadian psoriasis (p=0,792).
     
    Psoriasis is a chronic autoimmune disease with a prevalence of 2-3% of the entire world population. The BsmI VDR gene is one of the genes in intron 8 and has not been widely studied in the psoriasis group in Indonesia. This study aims to determine the effect of BsmI VDR gene polymorphisms and vitamin D plasma levels on the risk of psoriasis. This research method is a case control of 44 subjects in the psoriasis group and 44 subjects without psoriasis group with age> 18 years. PCR-RFLP was performed to isolate DNA and ELISA results in blood plasma. Data analysis was performed with the Chi Square test. The analysis results obtained the most female sex in both groups (cases: 56.8%; control: 72.7%) with an average age of 47 years in the case group and 24 years in the control group. GG (bb) variants were more common in both groups (cases: 77.3%; controls: 81.8%) than the GA (Bb) variant. In this study there were no AA (BB) variants. The mean vitamin D level in the case group was 25.92 ng/ml and the control group was 12.96 ng/ml. Plasma vitamin D levels in the case group (Deficiency: 36.4%; Insufficiency: 31.8%; Sufficiency: 31.8%) while in the control group (Deficiency: 81.8%; Insufficiency: 15.9%; Deficiency: 2.3%). It can be concluded that there is a significant relationship between plasma vitamin D levels and the incidence of psoriasis (p <0.05), conversely there is no significant relationship between variants of GG (bb) and GA (Bb) with the incidence of psoriasis (p = 0.792).
