    Peranan Nilai Skala Koma Glasgow (SKG) dan Gambaran Head CT–Scan Sebagai Prediktor Terhadap Perubahan Neurobehavior pada Penderita Trauma Kapitis Akut Ringan–Sedang

    Abstract
    Latar belakang : Trauma kapitis merupakan penyebab utama kematian dan kecacatan pada anak, dewasa dan pada usia produktif. Trauma kapitis juga dapat menyebabkan berbagai sequalae jangka pendek maupun jangka panjang meliputi gangguan kognitif, behavioral dan keterbatasan fisik. Penelitian ini bertujuan untuk mengetahui apakah nilai Skala Koma Glasgow (SKG) dan gambaran Head CT-Scan dapat menjadi prediktor terhadap perubahan neurobehavior pada penderita trauma kapitis akut ringan-sedang. Metode : Studi ini merupakan studi longitudinal dengan pengambilan sampel secara non random dengan metode konsekutif pada pasien trauma kapitis akut ringan-sedang yang dirawat di bangsal Neurologi RSUP. H. Adam Malik Medan periode 13 November 2006 sampai 15 Maret 2007. Dilakukan perhitungan nilai SKG dan dilakukan pemeriksaan Head CT-Scan, dan setelah penderita sadar dilakukan pemeriksaan terhadap perubahan neurobehavior dengan menggunakan Neurobehaviour Rating Scale (NRS). Untuk melihat pengaruh nilai SKG sebagai prediktor terhadap perubahan neurobehavior digunakan uji t–independen. Untuk melihat pengaruh gambaran Head CT-Scan sebagai prediktor terhadap perubahan neurobehavior digunakan uji oneway Anova. Hasil : Jumlah sampel trauma kapitis akut ringan-sedang sebanyak 61 orang, terdiri dari 40 orang laki-laki (80,3%) dan 21 orang wanita (19,7%). Berdasarkan nilai SKG, maka pada nilai rerata skor NRS tertinggi dijumpai pada kelompok SKG 9 – 12 sebesar 106,05 ± 30,48. Sedangkan nilai skor rata-rata NRS pada kelompok SKG 13 – 15 sebesar 41,90 ± 17,88 Terdapat perbedaan signifikan antara nilai rerata skor NRS pada kelompok SKG 9-12 dengan SKG 13-15 (p < 0,05). Berdasarkan gambaran Head CT-Scan, maka nilai rerata skor NRS tertinggi terdapat pada kelompok massive diffuse injury (135,00 ± 3,54), diikuti kelompok massive focal injury (116,50±10,21), mild/moderate diffuse injury (111,33 ± 20,03) medium focal injury (84,67 ± 13,32), serta mild focal injury (42,00 ± 21,21). Terdapat perbedaan signifikan antara nilai rerata skor NRS diantara berbagai kelompok gambaran Head CT Scan tersebut (p < 0,05). Kesimpulan : Nilai SKG dan gambaran Head CT-Scan dapat dijadikan sebagai prediktor terhadap perubahan neurobehavior pada penderita trauma kapitis akut ringan-sedang.
     
    Background : Head injury is the main cause of death and handicap in children, adults and age of productivity. Head injury can also cause various short term and long term sequele that covers cognitive disturbance, behavioural disturbance and physical limitation. This study was intended to determine whether Glasgow Coma Scale (GCS) and Head CT Scan image can be predictors toward neurobehavioral changes in acute mild-moderate head injury patients. Methode : This was a longitudinal study with non random consecutive sampling on acute mild-moderate head injuries patients admitted in neurology ward RS HAM in the periode of 13 November 2006 - 15 Maret 2007. GCS was evaluated and Head CT Scan was performed and after the patients were councious, neurobehavioral changes were examined using the Neurobehavioral Rating Scale (NRS). To determine the influence GCS as a predictor toward neurobehavioral changes, T-independent test was used. To determine the influence of head CT Scan image as a ptredictor toward neurobehaviotral changes, on-eway ANOVA was used. Result : The sample consisted of 61 patients with acute mild-moderate head injury, 21 were woman (19,7 %). Based on GCS, the highest mean NRS score was found in GCS 9-12 group, which was 106,05 ± 30,48. mean while, the mean of NRS score in GCS 13-15 group was 41,90 ± 17,88 (p< 0,05). There was a significant difference between the mean of NRS in GCS group 9-12 and group 13-15. Based on head CT- Scan image, the mean of the highest NRS score was found in massive diffuse injury (135,00 ± 3,54), followed by massive focal injury (116,50±10,21), mild/moderate diffuse injury (111,33 ± 20,03) medium focal injury (84,67 ± 13,32 and mild focal injury (42,00 ± 21,21). There was a significant difference between the mean of NRS among the group of Head CT-Scan images.(p< 0,05). Conclusion : Glasgow Coma Scale (GCS) and Head CT-Scan image can be used as a predictor toward neurobehavioral change in patients with acute mild-moderate head injury.
