dc.description.abstractBackground: Allergic contact dermatitis (ACD) is diagnosed by skin patch test. Cluster of differentiation 4+ (CD4+) and cluster of differentiation 8+ (CD8+) T cells have roles in allergic contact dermatitis pathogenesis. Study about correlation of CD4+ and CD8+ T cells with severity of patch test reactions is still limited. Objective: The aimed of the study is to investigate the correlation between levels of Cluster of differentiation 4+ (CD4+) and cluster of differentiation 8+ (CD8+) T cells in allergic contact dermatitis patients with severity of patch test reactions in the Division of the Immunodermatology, Department of Dermatology and Venereology, Haji Adam Malik general hospital Medan Methods: The design of the study is a cross sectional analytic. Sample was taken using consecutive sampling. A total of 45 patients with positive patch test in which the interpretation of the results was performed in accordance with the recommendations of International Contact Dermatitis Research Group, sera were collected by using FASCanto analysis to evaluate levels of CD4+ and CD8+ T cells. Then, the correlation between severity of patch test reactions and levels of CD4+ and CD8+ T cells were determined by using statistic analysis. Results: The most frequent allergen in this study is nickel (II) sulfate hexahydrate, potassium dichromate, Nisoprophyl- N-phenyl-4-phenylendiamine (IPPD), p- phenylendiamine (PPD), lanolin alchohol, in which the patients were predominantly female and the age group was ≤ 31 years. The mean of CD4+ and CD8+ T cells serum concentration in 45 patients with weak positive nickel (II) sulfate hexahydrate patch test are 883,91 sel/μL and 642,00 sel/μL respectively and with strong positive are 806,73 sel/μL and 715,93 sel/μL. Whereas with weak positive potassium dichromate patch test are 977,75 sel/μL and 743,25 sel/μL respectively, and with strong positive potassium dichromate patch test are 753,50 sel/μL and 839,50 sel/μL. Whereas with weak p-phenylendiamine (PPD) patch test are 833,00 sel/μL and 733,67 sel/μL sel/μL respectively and with strong positive PPD patch test are 1035,67 sel/μL and 914,67 sel/μL. Correlation test between levels of CD4+ and CD8+ T cells and severity of patch test reactions to nickel (II) sulfate hexahydrate, potassium dichromate and PPD were p ≥ 0,05. Conclusion: These findings suggest there is no correlation between level of CD4+ T cells and CD8+ T cells level with severity of patch test reactions in allergic contact dermatitis patients.en_US
dc.description.abstractLatar Belakang : Dermatitis Kontak Alergi (DKA) didiagnosis dengan pemeriksaan uji tempel. Sel T cluster of differentiation 4+ (CD4+) dan sel T cluster of differentiation 8+ (CD8+) merupakan salah satu faktor yang berperan dalam patogenesis DKA. Studi mengenai hubungan kadar sel T CD4+ dan sel T CD8+ dengan derajat kepositifan uji tempel masih sangat terbatas. Tujuan : Untuk mengetahui hubungan kadar sel T CD4+ dan sel T CD8+ dengan derajat kepositifan uji tempel pada serum penderita DKA di Divisi Imunodermatologi SMF Ilmu Kesehatan Kulit dan Kelamin RSUP H Adam Malik Medan Metode : Penelitian ini merupakan suatu studi analitik dengan rancang potong lintang (cross sectional), dimana cara pengambilan sampel penelitian dilakukan mengggunakan metode consecutive sampling. Pada empat puluh lima pasien yang telah dilakukan pemeriksaan uji tempel dan menunjukkan reaksi positif pada hasil pemeriksaan tersebut berdasarkan ketentuan International Contact Dermatitis Research Group, diambil serum darah dengan analisis FACSCanto untuk mengevaluasi kadar dari sel T CD4+ dan sel T CD8+. Kemudian, dengan menggunakan analisis statistik ditentukan hubungan antara kadar sel T CD4+ dan sel T CD8+ dengan derajat kepositifan uji tempel. Hasil : Alergen yang paling banyak ditemukan pada sampel penelitian ini adalah nickel (II) sulfate hexahydrate, potassium dichromate, N-isoprophyl-N-phenyl-4-phenylendiamine (IPPD), pphenylendiamine (PPD), lanolin alchohol, dan paling sering ditemukan pada wanita dan kelompok usia ≤ 31 tahun. Nilai rerata kadar sel T CD4+ dan sel T CD8+ pada 45 serum penderita DKA yang memiliki reaksi positif lemah terhadap nickel (II) sulfate hexahydrate pada penelitian ini masingmasing adalah 883,91 sel/μL dan 642,00 sel/μL, yang bereaksi positif kuat adalah 806,73 sel/μL dan 715,93 sel/μL. Sedangkan terhadap potassium dichromate yang memiliki reaksi positif lemah masing-masing adalah 977,75 sel/μL dan 743,25 sel/μL, yang bereaksi positif kuat adalah 753,50 sel/μL dan 839,50 sel/μL. Sedangkan terhadap PPD yang memiliki reaksi positif lemah masingmasing adalah 833,00 sel/μL dan 733,67 sel/μL, yang bereaksi positif kuat adalah 1035,67 sel/μL dan 914,67 sel/μL. Hasil uji korelasi antara kadar sel T CD4+ dan sel T CD8+ dengan derajat kepositifan uji tempel terhadap nickel (II) sulfate hexahydrate, potassium dichromate dan PPD didapatkan nilai p ≥ 0,05. Kesimpulan : Pada penderita DKA kadar sel T CD4+ dan kadar sel T CD8+ tidak memiliki hubungan yang signifikan dengan derajat kepositifan uji tempel.en_US
