    Pengaruh Ekstrak Biji Markisa Ungu (Passiflora Edulis Sims Var. Edulis) Terhadap Akne Vulgaris

    Abstract
    Background : Purple variant of passion fruit (Passiflora edulis Sims var. edulis) is a tropical plant that is highly consumed and used commercially in beverage industry. However, the seeds was not commonly used and disregarded as waste. Some studies reported that it has high content of linoleic acid and piceatannol, which have shown antibacterial, antiinflamatory and antioxidant activities. However, there is no report regarding its effect on Propionibacterium acnes. This bacteria play an important role in the pathogenesis of acne vulgaris, a chronic pilosebaceous inflammatory disorder that is varied in clinical manifestation from comedones to nodules or cysts. Objective : To determine the effect of topical use of passion fruit purple variant (Passiflora edulis Sims var. edulis) seeds extract on acne vulgaris. Methods : This is a quasi-experimental clinical trial with pretest-posttest design on 45 subjects with acne vulgaris. Diagnosis was made clinically and evaluations were done by counting noninflammatory, inflammatory, and total lesions of acne vulgaris, along with ultraviolet induced red fluorescence (UVRF) measurement before and after topical use of 10% passion fruit purple variant seeds extract cream at week 0, 2, 4, 6, 8. During the study, all adverse events reported were recorded and patient’s satisfaction rates were determined at week 8. Statistical analysis using dependent T-test and Wilcoxon test was done to evaluate the differences in acne lesion counts and UVRF before and after intervention, with p £ 0.05 indicates significance. This research has been approved by the Ethics Committee of Faculty of Medicine Universitas Sumatera Utara. Result : There were significant reductions in the noninflammatory (80,9%), inflammatory (71,1%) and total (73%) lesion count (p < 0.001) after 8 weeks of using passion fruit purple variant topically. In UVRF measurement, there were also significant reduction in spot count (36%) and percentage of area covered (45,9%) at the end of the study (p < 0.001). Only 1 out of 45 subjects (2.2%) experienced adverse event of mild and transient scaling. Most of the subjects (77,8%) noticed significant improvement on their acne vulgaris and expressed good satisfaction rate. Conclusion : Topical use of passion fruit purple variant seeds extract improves acne vulgaris condition with reduction in lesion counts and UVRF, that is well tolerated and gives good satisfaction rate on the patients.
     
    Latar Belakang : Markisa ungu (Passiflora edulis Sims var. edulis) merupakan tanaman tropis yang sering dikonsumsi dan digunakan secara komersial dalam industri minuman. Akan tetapi bijinya merupakan bahan tidak terpakai yang sering kali dibuang. Beberapa penelitian telah melaporkan kandungan asam linoleat dan piceatannol yang tinggi dalam biji markisa ungu, yang memiliki aktivitas antibakterial, antiinflamasi dan antioksidan. Akan tetapi, belum ada yang melaporkan mengenai efek ekstrak bijinya terhadap Propionibacterium acnes. Bakteri ini berperan penting dalam patogenesis akne vulgaris, suatu kondisi inflamasi kronis kulit pada unit pilosebasea dengan beragam manifestasi klinis dari komedo hingga nodul/kista. Tujuan : Untuk mengetahui pengaruh ekstrak biji markisa ungu (Passiflora edulis Sims var. edulis) terhadap perbaikan akne vulgaris. Metode : Penelitian ini merupakan uji klinis quasi-eksperimental dengan desain pretest-posttest pada 45 subjek dengan akne vulgaris. Diagnosis ditegakkan secara klinis dan evaluasi dilakukan dengan perhitungan jumlah lesi noninflamasi, inflamasi, dan total lesi serta pengukuran ultraviolet induced red fluorescence (UVRF) sebelum dan sesudah pemberian krim ekstrak biji markisa ungu 10% secara topikal pada minggu ke-0, 2, 4, 6, 8. Efek samping yang terjadi selama masa penelitian dicatat dan tingkat kepuasan pasien dinilai pada akhir penelitian. Uji-T berpasangan dan Wilcoxon dilakukan untuk melihat perbedaan sebelum dan sesudah intervensi dengan nilai p £ 0,05 dianggap bermakna. Penelitian ini telah mendapat persetujuan dari Komisi Etika Penelitian Kesehatan Fakultas Kedokteran Universitas Sumatera Utara. Hasil : Terdapat penurunan yang bermakna dari jumlah lesi noninflamasi, inflamasi dan total lesi akne vulgaris setelah 8 minggu pemberian ekstrak biji markisa ungu, dengan rerata penurunan sebesar 80,9%, 71,1%, dan 73% (p < 0,001), secara berurutan. Penurunan bermakna sebesar 36% dari jumlah spot dan 45,9% dari persentase area terjangkau UVRF juga diamati pada akhir penelitian (p < 0,001). Hanya 1 dari 45 subjek penelitian (2,2%) mengalami efek samping derajat ringan dan sementara berupa kulit terkelupas. Sebagian besar subjek penelitian (77,8%) menunjukkan tingkat kepuasan pasien yang baik. Kesimpulan : Penggunaan ekstrak biji markisa ungu secara topikal dapat mempengaruhi perbaikan akne vulgaris dan menurunkan jumlah lesi dan UVRF pasien akne vulgaris dengan efek samping yang ringan dan dapat ditoleransi serta tingkat kepuasan yang baik.
