    Hubungan Penggunaan Renal Resistive Index dan Central Venous Pressure Sebagai Prediktor Terjadinya Acute Kidney Injury pada Pasien Kritis di Ruangan ICU RSUP Haji Adam Malik Medan

    Abstract
    Introduction: The Doppler-based Renal Resistive Index (RI) changes before the change in the GFR level during both the development and the recovery processes of AKI. Renal RI has been shown to predict AKI. Central Venous Pressure (CVP) not only is a marker of resuscitation but also predominantly determines the microcirculatory perfusion pressure as an outflow obstruction. Objective: This study aims to determine the comparison of RI and CVP as a predictor of AKI at the Intensive Care Unit (ICU) of the Haji Adam Malik General Hospital Medan. Method: This study is using a prospective cohort method with a diagnostic test design. The study was conducted in the ICU room of the Adam Malik General Hospital, Medan in June-July 2020. The research subjects were 40 patients aged 18-65 years who underwent treatment that met the criteria for sepsis and septic shock, where the RI and CVP were examined when they entered the ICU. Result: Renal resistive index has better sensitivity and specificity than central venous pressure in predicting the incidence of AKI (sensitivity 68% vs 59%, specificity 77% vs 55.5%, ROC 0.870vs 0,321 (95% confidence interval)). Conclusion: Based on this study, renal resistive index has better sensitivity and specificity than central venous pressure in predicting incidence of AKI.
     
    Pendahuluan: Perubahan pada Renal Resistive Index (RI) berdasarkan Doppler terjadi sebelum perubahan terhadap Glomerulus Filtration Rate (GFR) selama perkembangan terjadinya Acute Kidney Injury (AKI) dan pada saat proses penyembuhan dari AKI. Central Venous Pressure (CVP) bukan hanya merupakan suatu penanda untuk resusitasi, tetapi juga dapat menentukan tekanan perfusi mikrosirkulatori sebagai obstruksi outflow. Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan pada RI dan CVP sebagai prediktor terjadinya AKI pada pasien kritis yang dirawat di Intensive Care Unit (ICU) Rumah Sakit Umum Pusat Haji Adam Malik. Metode: Penelitian menggunakan metode Cohort prospektif dengan desain uji diagnostik. Penelitian ini dilaksanakan di Rumah Sakit Umum Pusat Haji Adam Malik Medan pada bulan Juni-Juli 2020. Subjek penelitian adalah 40 pasien berusia 18-65 tahun yang menjalani perawatan yang memenuhi kriteria sepsis dan syok sepsis, dimana dilakukan pemeriksaan RI dan CVP saat masuk ke ruang ICU. Hasil: RI memiliki sensitivitas dan spesifisitas lebih baik dibandingkan central venous pressure dalam memprediksi kejadian AKI (sensitivitas 68% vs 59%, spesifisitas 77% vs 55,5%, ROC 0,870 vs 0,321 (interval kepercayaan 95%)). Kesimpulan: Berdasarkan hasil penelitian ini, Renal resistive index memiliki sensitivitas dan spesifisitas lebih baik dibandingkan central venous pressure dalam memprediksi kejadian AKI.
