    Peranan Neutrophil-To-Lymphocyte Ratio (NLR) Danfournier’s Gangrene Severity Index (FGSI) Sebagaifaktor Prediktor Mortalitas dan Korelasinya pada Pasien Fournier’s Gangrene

    Abstract
    Latar Belakang: Gangren Fournier merupakan penyakit yang relatif jarang, dengan angka kejadian penyakit 1,6 kasus per 100.000 pasien laki-laki dan angka kematian kasus 7,5%. Infeksi yang terjadi bersifat polymicrobial, yaitu kombinasi bakteri aerob dan anaerob. Tergolong penyakit yang berpotensi fatal dengan angka kematian yang tinggi dan termasuk dalam kasus gawat darurat di bidang urologi. Sistem penilaian cepat diperlukan untuk menilai parameter untuk memprediksi kematian pasien. Penilaian sederhana dan andal adalah FGSI dan NLR. Metode: Penelitian analitik dengan pendekatan cross-sectional, retrospektif dengan melihat data sekunder dari rekam medis dengan diagnosis Fournier Gangrene dari tahun 2015 sampai 2019 yang berobat ke Bagian Bedah, Bagian Urologi, RSUP Haji Adam Malik. Hasil: Sampel berjumlah 58 pasien, dengan angka kematian 24,13%. Terdapat hubungan yang signifikan antara skor FGSI dengan mortalitas dengan nilai cut off 8 dengan nilai sensitivitas 85,71% dan nilai spesifisitas 90,90%. Sementara itu, NLR juga ditemukan signifikan dengan nilai cut-off 8, dengan nilai sensitivitas 71,42% dan nilai spesifisitas 81,81%. Korelasi antara FGSI dan NLR ditemukan lemah dalam menentukan prognosis mortalitas (r = 0,353). Kesimpulan: Pada penelitian ini ditemukan prediktor yang signifikan untuk menentukan prognosis mortalitas yaitu FGSI dengan nilai cut off 9 dan NLR dengan nilai cut off 8, namun memiliki korelasi yang lemah antara kedua prediktor tersebut dalam menentukan prognosis kematian akibat penyakit Fournier Gangrene.
     
    Background: Fournier's gangrene is a relatively rare disease, with a disease incidence rate of 1.6 cases per 100,000 male patients and a case fatality rate of 7.5%. The infection that occurs is polymicrobial, which is a combination of aerobic and anaerobic bacteria. Classified as a potentially fatal disease with a high mortality rate and is included in the emergency case in the field of urology. A fast scoring system is needed to assess the parameters for predicting patient mortality. Simple and reliable assessments are FGSI and NLR. Methods: Analytical research with a cross-sectional, retrospective approach by looking at secondary data from medical records with a diagnosis of Fournier Gangrene from 2015 to 2019 who went to the Department of Surgery, Division of Urology, RSUP Haji Adam Malik. Results: There were 58 patients sampled, with the mortality rate is 24.13%. There is a significant relationship between the FGSI score and mortality with a cut-off value of 8 with a sensitivity value of 85.71% and a specificity value of 90.90%. At the same time, NLR was also found to be significant with a cut-off value of 8, with a sensitivity value of 71.42% and specificity value 81.81%. The correlation between FGSI and NLR was found to be weak in determining the prognosis of mortality (r = 0.353). Conclusions: In this study, significant predictors were found to determine the prognosis of mortality, namely FGSI with a cut-off value of 9 and NLR with a cut-off value of 8, but had a weak correlation between the two predictors in determining the prognosis of mortality from Fournier Gangrene disease.
