    LRINEC (Laboratory Risk Indicator for Necrotizing Fasciitis) Score sebagai Prediktor Terjadinya Nekrosis dan Perforasi pada Kasus Appendisitis Anak di RSUP H. Adam Malik Medan

    Abstract
    Pediatric Appendicitis Score has been used as a tool in assessing the risk of necrotizing fasciitis in cases of soft tissue infection. This study aims to determine the LRINEC (Laboratory Risk Indicator for Necrotizing Fasciitis) Score as a predictor of necrosis and perforation in cases of pediatric appendicitis in RSUP H. Adam Malik Medan. The study was prospective with patient data collection from January 2014 to December 2014. Patient data were collected including age, LRINEC (Laboratory Risk Indicator for Necrotizing Fasciitis) Score, and postoperative hystopathology. For 12 months, 30 patients with acute appendicitis who met the inclusion criteria obtained a strong correlation of Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) Score with operating findings, Pearson correlation coefficient (r) of .551 and p value 0.002 (0.002 <0.05, significance 0.05). With the LRINEC score cutoff ≥ 9 it has a positive predictive value of 78.5% (95% CI) and a negative predictive value of 62.5% (95% CI). The sensitivity itself is 64.7% and the specificity is 76.9%. This can be utilized in the management of preoperative and postoperative appendicitis patients in children.
     
    LRINEC (Laboratory Risk Indicator for Necrotizing Fasciitis) score telah digunakan sebagai alat bantu untuk mengetahui resiko terjadinya necrotizing fasciitis pada kasus-kasus infeksi soft tissue. Penelitian ini bertujuan untuk mengetahui LRINEC (Laboratory Risk Indicator for Necrotizing Fasciitis) score sebagai prediktor terjadinya nekrosis dan perforasi pada kasus appendisitis anak di RSUP H. Adam Malik Medan. Penelitian bersifat retrospektif dengan pengambilan data pasien periode Januari 2014 sampai Desember 2014. Data pasien dikumpulkan meliputi usia, LRINEC (Laboratory Risk Indicator for Necrotizing Fasciitis) score, dan temuan operasi. Selama 12 bulan, 30 pasien dengan appendisitis akut yang memenuhi kriteria inklusi didapatkan korelasi Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) Score dengan temuan operasi menggunakan korelasi Pearson, angka koefesien korelasi Pearson (r) sebesar .551 atau kuat serta dengan nilai p 0.002 (0.002 < 0,05, signifikansi 0,05). Dengan cutoff LRINEC score ≥ 9 memiliki nilai duga positif (positive predictive value) sebesar 78.5% (CI 95%) dan nilai duga negatif (negative predictive value) sebesar 62.5% (CI 95%). Sensitivitas sendiri sebesar 64.7% dan spesifisitas 76.9%. Hal ini dapat dimanfaatkan dalam manajemen preoperasi dan paska operasi pasien appendisitis pada anak.
