Perbedaan Pengaruh Parasetamol Infus Dengan Metamizol Injeksi terhadap Suhu Tubuh dan Outcome Pasien Stroke Akut Dengan Hipertermia

Abstract

Background: Hyperthermia has unfavourable effect for the outcome of acute stroke. The using of antipyretic could reduce body temperature, and was expected to improve the outcome of acute stroke. Objective: The aim of this study is to determine the difference between the effect of paracetamol infusion and metamizole injection in lowering the temperature of hypertermic acute stroke patient and its conjuction with the outcome at the fourteen day. Methods: This study was an experimental study with matching group pretest-post test design. The subjects were divided into 2 groups, the first was given paracetamol infusion 1000 mg (iv) and the other was given metamizole injection 1000 mg (iv), with 18 patients in each group. The measurement of body temperature was done three times, the first before administration of the drug, and then 1 and 3 hours after. The outcome was measured by NIHSS (≤ 5 = mild; 6-13 = moderate; > 13 severe) and mRS (1-2 = good; 3-6 = poor). Results: Either paracetamol infusion or metamizole injection, reduced body temperature significantly at one hour (p<0.001) and 3 hours (p<0.001) after administration of the drugs. There was no differences in effect for body temperature for the both of groups at one hour (p=0,091) and three hours (p=0,720) after the administration of antipyretic drugs. There was no significant effect in the improvement on outcome score NIHSS and mRS at 14 days after administration of paracetamol infusion or metamizole injection. Conclusions: This study suggested that paracetamol infusion and metamizole injection have an equal effect in reducing body temperature of acute stroke patients with hyperthermia but has no effect on the outcome.

Latar Belakang: Hipertermia memiliki efek yang tidak menguntungkan terhadap outcome pada penderita stroke akut. Penggunaan antipiretik dapat menurunkan suhu tubuh dan diharapkan dapat memperbaiki outcome penderita stroke akut. Tujuan: Tujuan dari penelitian ini adalah untuk mengetahui perbedaan pengaruh antara parasetamol infus dengan metamizol injeksi terhadap penurunan suhu tubuh pada pasien stroke akut dengan hipertermia dan kaitannya terhadap outcome pada hari ke 14. Metodologi: Penelitian ini studi eksperimental dengan matching group pretest-post test design. Terdiri dari 2 kelompok, yang pertama diberikan parasetamol infus 1000 mg (iv) dan yang kedua diberikan metamizol injeksi 1000 mg (iv), 18 pasien pada tiap kelompoknya. Pengukuran suhu tubuh dilakukan 3 kali, pertama sebelum pemberian obat, kemudian 1 dan 3 jam sesudahnya. Outcome diukur dengan NIHSS (≤ 5 = ringan; 6-13 = sedang; > 13 = berat) dan mRS (1-2 = baik; 3-6 = buruk ). Hasil: Baik parasetamol infus maupun metamizol injeksi, menurunkan suhu tubuh secara bermakna pada 1 jam (p<0,001) dan 3 jam (p<0,001) setelah pemberian obat. Tidak terdapat perbedaan efek yang bermakna terhadap suhu tubuh pada kedua kelompok pada 1 jam (p=0,091) dan 3 jam (p=0,720) setelah pemberian antipiretik. Tidak terdapat pengaruh bermakna pada perbaikan outcome NIHSS dan mRS hari ke-14 setelah pemberian parasetamol infus atau metamizol injeksi. Kesimpulan: Studi ini menunjukkan bahwa parasetamol infus dan metamizol injeksi menyebabkan penurunan suhu tubuh yang berimbang pada penderita stroke akut dengan hipertermia tapi tidak berpengaruh pada outcome.
