    Efektivitas Fototerapi Tunggal Dibandingkan Fototerapi Ganda pada Neonatus dengan Hiperbilirubinemia Indirek

    Abstract
    Background: Hyperbilirubinemia is one of the most common problems in term newborns and the phototherapy is the most widespread treatment for lowering bilirubin concentration in neonates. The double phototherapy unit could increase effectiveness of phototherapy. Objective: To compare the effectiveness of single phototherapy and double phototherapy in decreasing serum bilirubin and increasing spectral irradiance Methods: An open, randomized controlled trial was conducted at both H. Adam Malik Hospital Medan and Pirngadi Hospital Medan since June to December 2009. Subject divided into 2 group, one group received single phototherapy (n=30) and the other received double phototherapy (n=30). The criteria for inclusion in the study were term newborns with neonatal jaundice presenting in the first week of life. Serum bilirubin level and average spectral irradiation level measured at baseline and after12 h, 24 h of phototherapy. Results: The mean total bilirubin level of single and double phototherapy groups at the beginning of therapy were 17.7(SD1,45) and 17.5(SD1.34) mg/dL respectively, there was no significant difference between the values. After 12 hours of therapy the mean decrease in total serum bilirubin levels of single and double phototherapy group were 0.1 (SD 0.167) and 6.52 (SD 0.62) mg/dL respectively. The mean decreased in total serum bilirubin levels were significant differences between two groups (P<0.05). During the study period the sum of average spectral irradiance by double phototherapy was significantly higher than of the single phototherapy (P< 0.05). Conclusion: Our study showed that double phototherapy is more effective than single phototherapy in reduction of bilirubin in jaundiced newborns.
     
    Latar belakang: Hiperbilirubinemia merupakan salah satu dari banyak permasalahan pada bayi cukup bulan dan fototerapi merupakan terapi yang banyak digunakan untuk menurunkan kadar bilirubin pada bayi. Fototerapi ganda dapat meningkatkan efektivitas fototerapi. Tujuan: Untuk membandingkan efektivitas fototerapi tunggal dan fototerapi ganda dalam menurunkan kadar bilirubin dan meningkatkan spektrum iradiansi Metode: Uji klinis terbuka, dilakukan di 2 RS, RS.H.Adam Malik dan RS. Dr. Pirngadi Medan. Penelitian dimulai bulan Juni 2009 sampai Desember 2009., Sampel dibagi 2 grup secara acak sederhana. Satu grup dengan menggunakan fototerapi tunggal (n=30) dan drup yang ke 2 menggunakan fototerapi ganda (n=30). Kriteria inklusi pada penelitian ini adalah cukup bulan sesuai masa kehamilan yang secara klinis dijumpai ikterus atau kuning pada minggu pertama kehidupan. Serum bilirubin dan tingkat spektrum iradiansi diukur pada awal, 12 jam dan setelah 24 jam fototerapi. Hasil: Rerata kadar bilirubin awal pada ke dua grup yang menggunakan fototerapi ganda dan tunggal saat dimulai fototerapi adalah 17 mg/ dL, tidak ada perbedaan yang signikan antara ke 2 grup. Setelah 12 jam fototerapi terjadi penurunan kadar bilirubin pada grup fototerapi tunggal 0.1 (SD 0.167) dan grup fototerapi ganda 6.5 (SD 0.62) mg/dL. Penurunan rerata kadar serum bilirubin berbeda signifikan antara ke 2 grup (P<0.05). Selama penelitian spektrum iradiansi secara signifikan lebih tinggi pada grup fototerapi ganda dibandingkan fototerapi tunggal (P<0.05). Kesimpulan: Penelitian kami dapat bahwa fototerapi ganda lebih efektif dalam menurunkan kadar bilirubin pada bayi-bayi dengan hiperbilirubinemia dengan peningkatan spectrum iradiansi.
