    Perbandingan Penurunan Kadar Tromboksan A2 setelah Pemberian Thiamin Intravena dan NaCl 0,9% Intravena pada Pasien yang Menjalani Pembedahan dengan Anestesi Umum

    Abstract
    Pendahuluan: Thiamin adalah kofaktor penting untuk piruvat dehidrogenase; yang memfasilitasi konversi piruvat menjadi asetil-KoA, Defisiensi thiamin menyebabkan beberapa sindrom klasik termasuk beri-beri basah/wet beriberi (gagal jantung), beri-beri kering/dry beriberi (neuropati perifer), ensefalopati Wernicke, psikosis Korsakoff, dan beri-beri gastrointestinal. Tromboksan A2 (TxA2) termasuk dalam famili lemak yang disebut eikosanoid, merupakan metabolit asam arakidonat yang dihasilkan oleh 3 enzim penting yaitu fosfolipase A2, siklooksigenase-1 (COX-1)/siklooksigenase-2 (COX-2) dan tromboksan A2 sintetase (TXAS). TxA2 dilepaskan oleh trombosit dan sejumlah sel lainnya seperti makrofag, neutrofil dan sel endotel. TxA2 memiliki sifat protrombotik karena menstimulasi aktivitas trombosit dan agregasi trombosit. Tujuan: Untuk mengetahui pengaruh thiamin injeksi dalam menurunkan kadar tromboksan A2 dibandingkan NaCl 0,9% pada pasien yang menjalani pembedahan dengan anestesi umum. Metode: Penelitian ini merupakan penelitian uji klinis dengan desain double blind randomized controlled trial (RCT). Penelitian ini melibatkan 38 orang sampel, dan dikelompokan menjadi 2 kelompok, kelompok thiamin dan kelompok nacl 0,9 % kemudian dilakukan pengambilan darah sebanyak 6 ml pre dan post pemberian intervensi masing-masing sebanyak 1 ml setiap kelompok kemudian di cek kadar tromboksan. Hasil: Pada kelompok thiamin didapatkan nilai kadar trombosan sebelum 53,48 ± 72,49 dibandingkan dengan kadar tromboksan sesudah 32,49 ± 20,97 dengan selisih didapatkan mean -26,16. Namun secara statistik didapatkan nilai P = 0,573 (P> 0,05) Kesimpulan: Terdapat pengaruh pemberian thiamin dalam menurunkan kadar tromboksan pada pasien yang menjalani pembedahan dengan anestesi umum, namun secara statistik tidak didapatkan hubungan yang signifikan
     
    Introduction: Thiamin is an important cofactor for pyruvate dehydrogenase; Thiamin deficiency causes several classic syndromes including wet beriberi (heart failure), dry beriberi (peripheral neuropathy), Wernicke's encephalopathy, Korsakoff's psychosis, and beriberi. gastrointestinal. Thromboxane A2 (TxA2) belongs to a family of fats called eicosanoids, is a metabolite of arachidonic acid produced by 3 important enzymes, namely phospholipase A2, cyclooxygenase-1 (COX-1)/cyclooxygenase-2 (COX-2) and thromboxane A2 synthetase (TXAS). TxA2 is released by platelets and a number of other cells such as macrophages, neutrophils and endothelial cells. TxA2 has prothrombotic properties because it stimulates platelet activity and platelet aggregation. Objective: To determine the effect of thiamin injection in reducing thromboxane A2 levels compared to 0.9% NaCl in patients undergoing surgery under general anesthesia. Methods: This study is a clinical trial study with a double-blind randomized controlled trial (RCT) design. This study involved 38 samples, and were grouped into 2 groups, the thiamin group and the 0.9% nacl group, then 6 ml of blood was taken before and after the intervention, 1 ml each for each group and then checked for thromboxane levels. Results: In the thiamin group, the thromboxane level before was 53.48 ± 72.49 compared to the thromboxane level after 32.49 ± 20.97 with a mean difference of -26.16. However, statistically, the value of P = 0.573 (P> 0.05) Conclusion: There is an effect of thiamin administration in reducing thromboxane levels in patients undergoing surgery under general anesthesia, but there is no statistically significant relationship.
