    Kanker Serviks RSUP. H. Adam Malik Medan 2008-2012: Karakteristik Modalitas Terapi dan Luaran Pasien

    Abstract
    Objective : To determine the risk factors, characteristics and outcomes of patients with cervical cancer at the Department of Obstetrics and Gynecology FK.USU - Dr. H. Adam Malik Medan. Methods : The study design was an observational study with cross-sectional design (cross-sectional), performed at the Department of Obstetrics and Gynecology RSUP.H. Adam Malik Medan, began in January 2008 - December 2012. Result : Most cases of cervical cancer found in women> 40 years, with parity ≥ 4, low socio-economic conditions, generally, they have a history of vaginal discharge with histopathology result is squamous cell. Mostly found in stage IIIB and IIB, and is rarely found in earlier stages of disease, most measuring > 4 cm in tumor size and generally metastases does not occur. Most treatment of cervical cancer cases is chemoradiation, while the external pelvic radiotherapy is the most widely used. 5-year life survival of cervical cancer in stage I and II is greater than in stage III and IV as well as cases without lymph node involvement. Conclusion : To provide a higher life survival and provide a better therapeutic response, we should conduct cervical cancer case finding early. The handling of cases of cervical cancer with combination therapy should be able gain a better therapeutic response.
     
    Tujuan penelitian : Untuk Mengetahui profil faktor risiko, karakteristik dan luaran pasien kanker serviks di Departemen Obstetri dan Ginekologi FK.USU – RSUP. H. Adam Malik Medan. Desain penelitian : Rancangan penelitian ini merupakan penelitian observasional dengan desain potong lintang (cross sectional), yang dilakukan di Departemen Obstetri dan Ginekologi RSUP.H. Adam Malik Medan, dimulai pada Januari 2008 – Desember 2012. Hasil : Sebagian besar kasus dijumpai pada wanita >40 tahun, dengan paritas ≥4, kondisi sosial ekonomi rendah, umumnya mempunyai riwayat keputihan dan gambaran histopatologi umumnya skuamous sel. Sebagian besar ditemukan pada stadium IIIB dan IIB, sebagian besar berukuran >4 cm dan umumnya tidak terjadi metastase. Penanganan terbanyak dengan kemoradiasi, sedangkan jenis radioterapi yang paling banyak digunakan adalah eksternal pelvik radioterapi. Stadium I dan II memberikan harapan hidup dalam 5 tahun lebih besar dari pada stadium III dan IV demikian juga kasus tanpa keterlibatan kelenjar getah bening. Kesimpulan : Untuk memberikan harapan hidup lebih tinggi dan memberikan respon terapi lebih baik, sebaiknya melakukan penemuan kasus kanker serviks secara dini. Penanganan terhadap kasus kanker serviks sebaiknya dengan terapi kombinasi untuk mendapatkan respon terapi yang lebih baik.
