    Deteksi Mycobacterium leprae dengan Teknik Polymerase Chain Reaction dari Saliva Penderita Kusta

    Abstract
    Background: leprosy is a chronic infection disease, caused by Mycobacterium leprae, involved skin and peripheral nerves, but also other organs or system, except central nerves. Objective: to study PCR technique in detecting Mycobacterium leprae from saliva of leprosy patient. Method: 30 leprosy patient detected in study location were involved in this study. The diagnosis was established by clinical and bacteriological examination. Saliva was taken and DNA extraction was performed, nested primers (LP1-4) were used in the PCR procedure are specific to M. leprae species. Results: from 30 leprosy cases (6 PB and 24 MB) that include in this study, AFB was found in 62,5% patient. M. leprae was found in 10% patient which are performed PCR examination. In negative AFB patient group, M. leprae was found in 3,3% by PCR, whereas positif AFB patient group, M. leprae was found 6,7% by PCR. Conclusion: Saliva specimens not could be used for non invasive bacterial examination of leprosy, using PCR method, especially for PB leprosy patient.
     
    Latar belakang: Penyakit kusta adalah penyakit infeksi kronik yang disebabkan oleh Mycobacterium leprae yang menyerang saraf tepi, kulit dan jaringan tubuh lainnya kecuali susunan saraf pusat. Tujuan: mempelajari hasil pemeriksaan PCR dalam mendeteksi M. leprae dari saliva penderita kusta. Metode: 30 penderita kusta baru atau 1 – 3 bulan bulan pengobatan yang berobat di tempat penelitian, setelah dilakukan penentuan diagnosis klinis, pemeriksaan bakteriologis, dilakukan pengambilan saliva. Dilakukan ekstraksi DNA dari sampel saliva dan diperiksa PCR dengan menggunakan nested primer LP1-4 yang spesifik terhadap spesies M. leprae. Hasil: Dari 30 kusta yang diteliti (6 PB dan 24 MB), BTA ditemukan pada 62,5%. Setelah dilakukan pemeriksaan PCR, M. leprae ditemukan pada 10% penderita. Pada kelompok BTA negatif ditemukan M. leprae 3,3%, sedangkan pada kelompok BTA positif ditemukan M. leprae 6,7%. Kesimpulan: Sediaan saliva belum bisa dijadikan alternatif untuk pemeriksaan bakteriologik kusta walaupun bersifat non invasif guna pemeriksaan PCR, terutama pada penderita BTA negatif.
