    Hubungan Rinitis Alergi dan Disfungsi Tuba Eustachius dengan Menggunakan Timpanometri

    Abstract
    Background: Allergic rhinitis is a global health problem with increasing prevalence and can impact on the quality of life of sufferers. One of co-morbidity of allergic rhinitis is otitis media which closely related to Eustachian tube dysfunction associated with middle ear pressure. Objective: To determine the relationship of allergic rhinitis with Eustachian tube dysfunction using tympanometry. Methods: Case-control studies, Tubal dysfunction determined with tympanometry (MEP negative / <-25 Dapa) and tubal function test results were not good (≤15 daPa). Chi-square test and logistic regression analysis used for data analysis. Results: There were 60 samples with 73.3% of women with the highest age group is 21-30 years of age (50%) with a mean age of 29.33 years. The most commonest of classification ARIA-WHO was moderate-severe persistent allergic rhinitis were 36.7% followed by mild intermittent allergic rhinitis was 16.7%. The most types of aero-allergens are mites 96.6% and house dust mites 90.0%. The most tympanogram type in both groups were type A, which case group was 73.3% (mean -40.8 daPa MEP), type As 10%, type ad and type B respectively 3.3%, type C 10.0% and control groups type A 93.3%, type As 6.7%. At case group of 83.3% of the tubal function test abnormal and normal 16.7%, while the control group is 93.3% normal and 6.7% abnormal. Chi-square test showed a significant difference between the type of intermittent and persistent allergic rhinitis with Eustachian tube dysfunction (p = 0.006) and also allergens types with Eustachian tube (p= 0.0041). Multivariate test revealed allergic rhinitis type contributed Eustachian tube dysfunction OR:2.9 (95% CI 1.52-241.90) p=0.022, neither allergen type with Eustachian tube dysfunction OR:2.2 (95% CI 0.01-7.31) p=0.295. Conclusion: Allergic rhinitis, more common which three times contributes a dysfunctional Eustachian tubes than the non-allergic rhinitis.     Pendahuluan: Rinitis alergi merupakan masalah kesehatan global dengan prevalensi yang terus meningkat serta dapat berdampak pada penurunan kualitas hidup penderitanya. Ko-morbiditas rinitis alergi salah satunya adalah otitis media yang sangat erat hubungannya dengan gangguan fungsi tuba Eustachius yang berkaitan dengan tekanan telinga tengah Tujuan: Mengetahui hubungan rinitis alergi dengan disfungsi tuba Eustachius dengan menggunakan timpanometri. Metode: Studi kasus-kontrol (case-control). Disfungsi tuba ditentukan dengan pemeriksaan timpanometri (MEP negatif/<-25 daPa) dan hasil tes fungsi tuba yang tidak baik. Analisis hasil dengan uji Chi-square dan Analisis regresi logistik. Hasil: Didapatkan 60 sampel dengan jumlah perempuan 73.3% dengan kelompok umur terbanyak adalah usia 21-30 tahun (50% ) dengan rerata umur 29.33 tahun. Klasifikasi ARIA-WHO yang paling banyak yaitu rinitis alergi persisten sedang berat 36.7% dan yang paling sedikit adalah rinitis alergi intermitten ringan 16.7%. Jenis alergen inhalan terbanyak yaitu tungau 96.6% dan tungau debu rumah 90.0%. Tipe timpanogram yang terbanyak pada kedua kelompok yaitu tipe A, dimana kelompok kasus dengan tipe A 73.3% (rerata MEP -40.8 daPa), tipe As 10%, tipe Ad dan tipe B masing-masing 3.3%, tipe C 10.0% dan kelompok kontrol tipe A 93.3% dan tipe As 6.7%. Pada kelompok kasus 83.3% hasil tes fungsi tuba abnormal dan 16.7% normal, sedangkan kelompok kontrol yaitu 93.3% normal dan 6.7% abnormal. Uji chi-square menunjukkan perbedaan yang signifikan antara tipe rinitis alergi intermitten dan persisten dengan disfungsi tuba Eustachius (p=0.006) dan perbedaan yang signifikan antara jenis alergen indoor dan outdoor dengan disfungsi tuba Eustachius (p=0.041). Hasil uji multivariat terdapat bahwa tipe rinitis alergi berpengaruh tiga kali lebih besar terhadap disfungsi tuba Eustachius (OR 2.9; 95% IK: 1.52-241.90 dan nilai p=0.022) sedangkan jenis alergen dua kali lebih berpengaruh terhadap disfungsi tuba dengan OR 2.2; 95% IK: 0.01-7.31 dan nilai p=0.295. Kesimpulan: Rinitis alergi, 3 kali lebih sering mengalami disfungsi tuba Eustachius dari pada non rinitis alergi. OR:2.5 (95% CI 2.36-2.99) P=0.000.
