    Perbedaan Kadar Pepsin dari Saliva pada Penderita Refluks Laringofaring (LPR) dan Non-LPR yang Didiagnosis Berdasarkan Reflux Symptom Index (RSI) dan Reflux Finding Score (RFS)

    Abstract
    Background: Diagnosis of laryngopharyngeal reflux (LPR) currently used as a gold standard is pH monitoring examination. However, it is still far from ideal criteria due to its sensitivity reported only in 50% to 80%, and approximately 12% of ENT patients can not tolerate this procedure. The latest study to detect LPR is by determining the existence of pepsin within the larynx using immunoasssay method (ELISA). Objective: To determine the differences in levels of pepsin between patients with LPR and non LPR. Materials and Methods: An analytic cross-sectional study was conducted on 20 patients with LPR (values of RSI >13 and RFS >7), and 20 patients with non-LPR (values of RSI <13 and RFS <7) as the comparison, from February 2016 until Desember 2016. Physical examination and laboratory tests were enroled for all samples. Results: A highly significant correlation was observed between the LPR group and non LPR group, with levels of pepsin obtained was p=0.024 (p <0.05), in which levels of pepsin in LPR group were higher (mean values 0.783 ± 0.393) compared to non LPR group (mean values 0.591 ± 0.216). Conclusion: The examination of pepsin in saliva can be used for the diagnosis of LPR.
     
    Latar Belakang: Diagnosis dari laryngopharynngeal reflux (LPR) sampai saat ini digunakan sebagai gold standard adalah pemeriksaan pH monitoring, namun pemeriksaan ini masih jauh dari kriteria ideal oleh karena sensitivitasnya dilaporkan hanya 50% sampai 80% dan sekitar 12% dari pasien THT tidak dapat mentoleransi prosedur ini. Penelitian terbaru untuk mendeteksi LPR adalah dengan menentukan ada tidaknya pepsin pada laring dengan metode immunoasssay (ELISA). Tujuan: Mengetahui perbedaan kadar pepsin pada penderita LPR dan non LPR. Bahan dan Metode: Penelitian analitik dengan pendekatan cross-sectional ini dilakukan terhadap 20 pasien LPR dengan nilai RSI >13 dan RFS >7 dan 20 pasien non-LPR dengan nilai RSI <13 dan RFS <7 sebagai pembanding dari Februari 2016 hingga Desember 2016. Dilakukan pemeriksaan klinis dan laboratorium terhadap semua sampel tersebut. Hasil: Terdapat hubungan yang sangat bermakna antara kelompok LPR dan Non LPR dengan kadar pepsin diperoleh nilai p=0,024 (p<0,05). Dimana, kadar pepsin pada kelompok LPR lebih tinggi dengan nilai rerata (0,783 ± 0,393) dibanding kelompok Non LPR dengan nilai rerata (0,591 ± 0,216). Kesimpulan: Pemeriksaan pepsin dari saliva dapat digunakan dalam menegakkan diagnosis LPR.
