    Hubungan Skala Stres dan Kadar Kortisol Serum dengan Skor Psoriasis Area and Severity Index (PASI) pada Pasien Psoriasis Vulgaris

    Abstract
    Background: Psoriasis is a chronic, inflammatory and proliferative conditions of the skin, characterized by keratinocytic hyperproliferation. It is one of the most illustrative examples of the close relation between exacerbation or severity of disease and the psychologic burden of the patients. In recent years, the association of neuroendocrine system with psoriasis has been a matter of interest for many researchers as it is thought to be an immune-mediated disorder that flares up by psychoemotional stress. Objective: To establish the correlation between stress scale and serum cortisol level with Psoriasis Area and Severity Index (PASI) scores in psoriasis vulgaris patients. Methods: This is a cross-sectional correlation analytic study. This study was performed since September 2016 until October 2017 in Haji Adam Malik General Hospital Medan, involving 33 psoriasis vulgaris patients. The quantitative measurement of cortisol in serum was obtained from enzyme-linked immunosorbent assay (ELISA). The stress scale was assessed through The Social Readjustment Rating Scale questionnaire. The Pearson’s correlation test was used to determine the correlation between two variables, followed by multivariate analysis. Results: Stress scale and PASI score are strongly positive correlated with correlation coefficient 0,742 while serum cortisol level and PASI score indicates a moderate negative correlation with correlation coefficient -0,524. Multiple linear regression analysis revealed that stress scale has the strongest effect on PASI scores, with the linear regression model is Y (PASI score) = 0,310+0,121X (stress scale) Conclusion: Stress may aggravate psoriasis and correlate to the severity of the psoriasis.
     
    Latar Belakang: Psoriasis adalah kondisi kulit inflamasi dan proliferatif yang berlangsung kronik, ditandai oleh hiperproliferasi keratinosit. Psoriasis merupakan salah satu contoh yang paling mewakili dari hubungan erat antara eksaserbasi ataupun keparahan penyakit dengan beban psikologis pasien. Dalam beberapa tahun terakhir, hubungan sistem neuroendokrin dengan psoriasis menarik minat banyak peneliti karena psoriasis diyakini sebagai penyakit diperantarai-imun yang dapat dicetuskan oleh stres. Tujuan: Untuk mengetahui hubungan antara skala stres dan kadar kortisol serum dengan skor PASI pada pasien psoriasis vulgaris Metode: Penelitian ini adalah penelitian analitik korelasi dengan pendekatan potong lintang. Penelitian dilakukan sejak September 2016 hingga Oktober 2017 di RSUP H. Adam Malik Medan dengan mengikutsertakan 33 pasien psoriasis vulgaris. Kadar kortisol dalam serum subyek diperiksa dengan metode enzyme-linked immunosorbent assay (ELISA). Skala stres dinilai melalui pengisian kuesioner The Social Readjustment Rating Scale. Digunakan uji korelasi Pearson untuk mengethui korelasi antar variabel, dan dilanjutkan dengan analisis multivariat. Hasil: Hubungan antara skala stres dengan skor PASI adalah positif kuat, dengan nilai koefisien korelasi (r) 0,742. Hubungan antara kadar kortisol serum dengan skor PASI adalah negatif sedang, dengan nilai koefisien korelasi (r) -0,524. Dari hasil analisis regresi linear berganda ditemukan bahwa skala stres merupakan variabel yang paling berpengaruh terhadap skor PASI, dengan persamaan regresi linear: Y (skor PASI) = 0,310+0,121X (skala stres) Kesimpulan: Stres dapat memperburuk psoriasis dan berkorelasi dengan keparahan psoriasis.
