dc.description.abstractBackground : Diabetes is a risk factor for stroke and related to the severity of functonal outcome after stroke. The management of diabetes with the glycohemoglobin (HbA1c) as the indicator have the strong influence to the functional outcome after stroke. Diabetes is also the risk factor for developing of kidney disease which is associated to the functional outcome after stroke. Objective: The objective of this study is to analyze the association between the level of creatinin clearance (CrCl) as the indicator of kidney disfunction, and the level of HbA1c with functional outcome after acute stroke in diabetic patients. Methods: This is the cross sectional observational study of the acute phase of stroke in the diabetic patients. The level of HbA1c and creatinine clearance were measured in all samples. All CrCL levels were categorized into the stage of Glomerular Filtration Rate (GFR). The functional outcomes were measured in the day 14 after admission with the score of Barthel Index, National Institutes of Health Stroke Scale (NIHSS) and modified Rankin Scale (mRS). Results: Thirty-four patients of diabetic patients with stroke were enrolled in this study, 18 males (52,9%) and 16 females (47,1%). There were 15 (44,2%) subjects aged between 51-60 years old and 5 subjects aged between 71-80 years old. The level of HbA1c > 10% was found in 13 subjects (38,2%) and 2 subjects (5,9%) with thelevel of HbA1c < 7%. From the value of CrCl, there were 16 (47,1%) patients diagnosed with chronic kidney disease (CKD) as the GFR cut off point <60. From the measurement of functional outcomes on the 14th day, there were 21 (61,8%) patients had BI score between 0-55, 22 (64,7%) patients had NIHSS score between 6-13 and 16 (47,1%) patients had mRS score 3. There was no significant association between the value of CrCL and the value of HbA1c with the functional outcomes. Conclusion: In this study we found the association between the functional outcome with the creatinine clearance and the level of HbA1c in the diabetics patients with stroke but the association was not significant.en_US
dc.description.abstractLatar Belakang : Diabetes sebagai faktor resiko stroke berhubungan dengan perburukan outcome fungsional paska stroke iskemik. Nilai Glycohemoglobin (HbA1c) sebagai indikator penatalaksanaan diabetes berpengaruh terhadap outcome fungsional paska stroke. Diabetes juga merupakan faktor resiko timbulnya gangguan fungsi ginjal dimana gangguan fungsi ginjal sendiri berhubungan terhadap outcome fungsional paska stroke. Tujuan : Penelitian ini bertujuan untuk mengetahui apakah ada hubungan antara nilai creatinine clearance (CrCl) sebagai indikator kerusakan ginjal dan nilai HbA1c dengan outcome fungsional pada penderita stroke iskemik fase akut dengan diabetes. Metode : Penelitian ini merupakan studi observasional secara potong lintang (cross sectional) terhadap penderita stroke iskemik fase akut dengan diabetes. Pada seluruh sampel dilakukan pemeriksaan laboratorium lengkap, nilai HbA1c, perhitungan nilai CrCl yang dikelompokkan menurut stage dari Glomerulus Filration Rate (GFR), dan dilakukan penilaian outcome fungsional pada hari ke-14 menggunakan skor Barthel Index (BI), National Institutes of Health Stroke Scale (NIHSS) dan modified Rankin Scale (mRS). Hasil : Tigapuluh empat orang subjek yang diteliti terdiri dari 18 orang laki-laki (52,9%) dan 16 orang perempuan (47,1%). Sampel terbanyak pada kelompok antara 51 - 60 tahun sebanyak 15 orang (44,2%),dan yang paling sedikit adalah kelompok usia 71 - 80 tahun sebanyak 5 orang (14,7%). Sebanyak 13 orang (38,2%) memiliki nilai HbA1c > 10% dan 2 orang (5,9%) dengan HbA1c <7%. Pada hasil pemeriksaan CrCL, dijumpai sebanyak 16 pasien (47,1%) menderita CKD (sesuai cut off nilai GFR < 60). Setelah hari ke-14 , skor BI terbanyak dijumpai pada skor 0-55 yaitu 21 orang (61,8%), skor NIHSS terbanyak dijumpai pada skor 6-13 yaitu 22 orang (64,7%) dan pada mRS,skor mRS terbanyak dijumpai pada skor 3 yaitu 16 orang (47,1%). Tidak dijumpai hubungan yang signifikan antara nilai CrCL dan nilai HbA1c dengan outcome fungsional. Kesimpulan : Pada penelitian ini dijumpai hubungan antara nilai CrCl dan nilai HbA1c dengan outcome fungsonal pada penderita stroke iskemik dengan diabetes namun hasilnya tidak signifikan.en_US
