    Hubungan antara Kadar Magnesium dan Ferritin Serum dengan Frekuensi, Durasi dan Intensitas Migren

    Abstract
    Background: Magnesium is an essential substance in many processes which have an essential role in the cell. Magnesium dediciency can trigger hiperagregasi platelet, damage serotonin receptors, and increases the affinity of NMDA receptors. Accumulation of excessive iron can directly cause dysfunction of the thalamus, and do not directly target the brain more susceptible to oxidative stress. Objective: This study was aimed to determine the relation of serum magnesium and ferritin level with frequancy, duration, and intensity of migraine. Methods: This study was a cross sectional study conducted on patients with migraine who went to the sefalgia outward of Neurology departement, RSUP H. Adam Malik hospital from September to November 2014. Migraine patients who met the inclusion criteria was anamnesis for filling the questionnaire and then performed vein blood tests to measure the levels of serum magnesium and ferritin. Results: There are 30 people with migraine in this study, consisting of 25 persons (83%) of women and 5 persons (17%) were male, with mean age was 31,37±9,71 years. Median and range of serum magnesium levels was 1,89 mEq/L (1,62 to 9,07) and the mean±SD serum ferritin levels were 102,371±62,624 ng/L. The subjects experienced a mean frequency was 4,1±3,57 times per month, a mean±SD of intensity of migraine were6,74±1,7 by using the VAS, and duration of migraine most are over 20 years (66,7%). There is a significant negative correlation between serum magnesium levels and the frequency and intensity of migraine with each value are r= -o,473; p= 0,008 and r= -0,673; p= <0,001, whereas the correlation between serum magnesium levels with duration was not significant (r= 0,134; p= 0,481). There was insignificant correlation between serum ferritin levels with the frequency, duration and intensity of migraine, with each value is r= -0,03, p= 0,874; r= 0,022, p= 0,909 and r= 0,053, p= 0,782. Conclusion: There was a significant correlation between serum magnesium levels with frequency (r = -0,473, p = 0,008) and intensity of migraines (r = -0,673, p = <0,001).
     
    Latar Belakang: Magnesium merupakan zat essensial dalam banyak proses yang mempunyai peranan essensial pada sel. Kekurangan magnesium dapat mencetuskan hiperagregasi platelet, merusak reseptor serotonin, dan meningkatkan afinitas reseptor NMDA. Akumulasi besi yang berlebihan dapat secara langsung menyebabkan disfungsi talamus, dan tidak langsung menjadikan otak lebih rentan terhadap stres oksidatif. Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan antara kadar magnesium dan ferritin serum dengan frekuensi, durasi dan intensitas migren Metode: Penelitian ini merupakan studi potong lintang yang dilakukan terhadap pasien migren yang berobat ke poliklinik sefalgia bagian Neurologi RSUP H. Adam Malik Medan periode September hingga November 2014. Penderita migren yang memenuhi kriteria inklusi dianamnesis untuk pengisian kuisioner kemudian dilakukan pemeriksaan darah vena untuk mengukur kadar magnesium dan ferritin serum. Hasil: Terdapat 30 orang penderita migren dalam penelitian ini, terdiri dari 25 org (83%) perempuan dan 5 org (17%) laki-laki, dengan rerata usia subyek adalah 31,37±9,71 tahun. Median dan rentang kadar magnesium serum adalah 1,89 mEq/L (1,62 – 9,07) dan rerata±SD kadar ferritin serum adalah 102,371±62,624 ng/L. Subyek mengalami rerata frekuensi adalah 4,1±3,57 kali per bulan dengan rerata±SD intensitas migren adalah 6,74±1,7 dengan menggunakan VAS, dan durasi migren terbanyak adalah lebih dari tahun yaitu 20 org (66,7%). Terdapat korelasi negatif yang signifikan antara kadar magnesium serum dengan frekuensi dan intensitas migren dengan masing-masing nilai adalah r = -0,473; p = 0,008 dan r = - 0,673; p = <0,001, sedangkan korelasi antara kadar magnesium serum dengan durasi tidak signifikan (r = 0,134; p = 0,481). Terdapat korelasi yang tidak signifikan antara kadar ferritin serum dengan frekuensi, durasi maupun intensitas migren, dengan masing-masing nilai adalah r = -0,03, p = 0,874; r = 0,022, p = 0,909, dan r = 0,053; p = 0,782. Kesimpulan: Terdapat korelasi yang signifikan antara kadar magnesium serum dengan frekuensi (r = -0,473, p = 0,008) dan intensitas migren (r = - 0,673, p = <0,001).
