    Hubungan Stage dengan Tumor Presenting Score (TPS) Programmed Death Ligand-1 pada Pasien Adenokarsinoma Paru di Kota Medan 2018

    Abstract
    Background : Lung cancer is a problem and one of the causes of death from malignancy. Many therapies have been developed to overcome this disease by Surgery and medicamentosa therapies. And medicamentosa therapies are important role in the treatment of malignancies because most lung cancers are diagnosed after an advanced stage, so many drugs are developed, one of which is immunotherapy. That is the blocking of the programmed death ligand 1 (PD-L1) checkpoint receptor. Methods : This study was a category description study on 35 paraffin block specimens that had been diagnosed with pulmonary adenocarcinoma. All paraffin blocks were reassessed in the anatomic pathology laboratory and 35 specimens were found that fit the inclusion criteria. All suitable paraffin blocks are colored with Dako PD-L1 IHC 22C3 pharmDx. and was read by two pathologists by the blind method. Results : in this study, the range of ages‟ patients was 40-60 years, with the most sex being male as many as 31 patients (88.6%). Smoking status with mild IB was 16 patients (45.7%), and advanced clinical stage (IV) was 33 patients (82.5%). Conclusion: That the expression of PD-L1 will be expressed in heavy smokers and high stage of lung cancer.
     
    Latar Belakang : Kanker paru merupakan masalah dan salah satu penyebab kematian dari keganasan. Banyak terapi yang telah dikembangkan untuk mengatasi penyakit ini dengan cara medikamentosa dan tindakan invasif. Dan terapi medikamentosa sangat berperan dalam penanganan keganasan yang dikarenakan kebanyakan kanker paru terdiagnosis setelah stage lanjut sehingga banyak obat-obatan yang dikembangkan salah satunya dengan imunoterapi dengan cara pemblokadean reseptor checkpoint programmed death ligand 1 (PDL1). Metode : Penelitian ini merupakan studi deskripsi analitik terhadap 52 spesimen blok parafin yang telah didiagnosa adenokarsinoma paru di Rumah Sakit H. Adam Malik Medan. Semua blok parafin dinilai ulang di laboratorium patologi anatomi Rumah Sakit Dharmais Jakarta dan didapatkan 35 spesimen yang sesuai dengan kriteria inklusi. Spesimen tersebut diwarnai dengan Dako PD-L1 IHC 22C3 pharmDx. dan dibacakan oleh dua orang patolog dengan metode blind. Hasil : Pada penelitian ini, rentang umur pasien adalah berumur 40-60 tahun, dengan jenis kelamin terbanyak adalah laki-laki yaitu sebanyak 31 pasien (88,6%). Status merokok dengan IB ringan sebanyak 16 pasien (45,7%), dan stage klinis yang lanjut (IV) sebanyak 33 pasien (82,5%). Kesimpulan : Ekspresi PD-L1 akan terekspresi pada perokok berat dan stage yang tinggi.
