dc.description.abstractBackground In liver cirrhosis, impaired insulin sensitivity and subsequent alteration in glucose metabolism, such as high prevalence of insulin resistance and glucose intolerance are reported and adiponectin level positively correlated with insulin sensitivity and negatively correlated with serum transaminase levels. Several study before reported that presence of high plasma adiponectin levels in patients with liver cirrhosis relative to the control and these level increased in proportion with the severity of liver cirrhosis. Objective We want to determine the difference of plasma adiponectin levels in patients with liver cirrhosis according to Child Pugh classification. Method The research had been done since Maret 2010 until July 2010 with cross sectional method. The amount of participant examined 30 patients with liver cirrhosis which outpatient in clinic and hospitalized in Adam Malik hospital. Of all patients were performed anamnese, physical diagnostic, Ultrasonography, laboratory examination, and plasma adiponectin levels. Plama adiponectin concentrations according to child pugh score were analyzed by anova. Results Plasma adiponectin concentrations were increased in patients with liver cirrhosis relative to its normal value (12,93 ± 6,99 μg/ml vs 5-10 μg/ml) and this level increased in proportion with the severity of liver cirrhosis ( Child A : Child B : Child C = 2,6±1,1 μg/ml : 9,8±3,2 μg/ml : 17,5±5,6 μg/ml, p<0,001) and negative correlation between plasma adiponectin and BMI, albumin and fasting glucose and positive correlation between plasma adiponectin and total bilirubin, AST Conclusion Plasma adiponectin concentrations were increased in patients with liver cirrhosis and this level increased in proportion with the severity of liver cirrhosisen_US
dc.description.abstractLatar Belakang Pada Sirosis Hati dilaporkan terjadi gangguan sensitifitas insulin diikuti dengan perubahan metabolisme glukosa seperti tingginya prevalensi resistensi insulin dan intoleransi glukosa, dan adiponektin secara langsung berkorelasi dengan sensitifitas insulin dan berkorelasi secara negatif dengan kadar transaminase serum, namun studi sebelumnya menyebutkan bahwa kadar adiponektin plasma meningkat pada Sirosis hati dan peningkatan ini sejalan dengan semakin memberatnya tingkat kerusakan sirosis hati. Tujuan Untuk mengetahui perbedaan kadar adiponektin plasma pada penderita Sirosis Hati sesuai dengan tingkat keparahan menurut kriteria Child Pugh Metode Penelitian dilakukan mulai bulan Maret 2010 sampai dengan Juli 2010 dengan metode potong lintang. Jumlah peserta yang diperiksa adalah 30 orang penderita Sirosis hati yang berobat rawat jalan di poliklinik dan rawat inap Gastroentero Hepatologi RSHAM. Semua penderita dilakukan anamnesa, pemeriksaan fisik, USG, pemeriksaan laboratorium, dan adiponektin plasma. Kadar adiponektin berdasarkan kriteria Child Pugh diuji dengan Anova. Hasil Kadar adiponektin meningkat pada pasien sirosis dibanding dengan nilai normalnya (12,93 ± 6,99 μg/ml vs 5-10 μg/ml), dan adiponektin plasma meningkat sejalan dengan meningkatnya tingkat kerusakan hati ( Child A : Child B : Child C = 2,6±1,1 μg/ml : 9,8±3,2 μg/ml : 17,5±5,6 μg/ml, p<0,001) serta didapatkan korelasi negatif antara adiponektin dengan IMT, albumin dan KGD puasa serta korelasi positif dengan bilirubin total dan SGOT Kesimpulan Kadar adiponektin plasma meningkat pada pasien Sirosis hati dan peningkatan ini sejalan dengan tingkat kerusakan hati.en_US
