Profil Skor Nugent Berdasarkan Pewarnaan Gram pada Pasien Vaginosis Bakterial di RSUP Haji Adam Malik Medan

Abstract

Introduction Bacterial vaginosis is a clinical syndrome due to exchange of Lactobacillus sp. normal flora of vagina with various type of microorganism Gram (+) or Gram (-), such Gardnerella vaginalis, Mobilluncus sp., Bacteroides sp., dan Mycoplasma. Nugent score in Gram staining is gold standard to diagnose a bacterial vaginosis. Objective To indentify Nugent skor profile based on Gram staining in bacterial vaginosis patient at Haji Adam Malik General Hospital Medan. Method This is a the descriptive study with cross sectional design. Sixty two bacterial vaginosis patient which comes to dermatologist division and obstetric and gynecology division at Haji Adam Malik General Hospital. All of them were examined Nugent score based on Gram staining. Result Nugent score total based on Gram staining 100% for 7-10 score. The highest morphotype score for Lactobacillus were and 51,61% for 4 score, and the lowest were 61% for 0 score. The highest morphotype score for Gardnerella and anaerob were 79% for 4 score and the lowest were 2% for 1 score. The highest morphotype score for Mobilluncus were 49,99% for 1 score and the lowest were 4,8% for 0 score. Conclusion This study shows that Lactobacillus were dominance as normal flora of vagina siginificantly decreased in bacterial vaginosis patient.

Latar Belakang Vaginosis bakterial adalah sindrom klinik akibat pergantian Lactobacillus sp. sebagai flora normal vagina dengan berbagai jenis mikroorganisme Gram positif maupun Gram negatif, seperti : Gardnerella vaginalis, Mobilluncus sp., Bacteroides sp., dan Mycoplasma. Skor Nugent pada pewarnaan Gram adalah baku emas untuk mediagnosis vaginosis bakterial. Tujuan Untuk mengetahui profil skor Nugent berdasarkan pewarnaan Gram pada pasien vaginosis bakterial di RSUP. H. Adam Malik Medan Metode Penelitian ini merupakan penelitian deskriptif dengan pendekatan pontong lintang (cross sectional). Enam puluh dua orang pasien vaginosis bakterial yang berobat ke poliklinik Ilmu Kesehatan Kulit dan Kelamin dan poliklinik Ilmu Kebidanan dan Penyakit Kandungan RSUP. H. Adam Malik Medan. Terhadap subjek penelitian dilakukan anamnesis dan pemeriksaan klinis dan kemudian dilakukan pemeriksaan skor Nugent berdasarkan pewarnaan Gram. Hasil Total skor Nugent berdasarkan pewarnaan Gram adalah 100% untuk skor 7-10. Skor morfotipe terbesar bakteri batang Gram positif besar (Lactobacillus) adalah 51,61% untuk skor 4, dan terkecil adalah 1,61% untuk skor 0, skor morfotipe terbesar bakteri batang Gram negatif kecil dan variabel (Gardnerella dan anaerob) adalah 79% untuk skor 4 dan terkecil adalah 2% untuk skor 1. Skor morfotipe terbesar untuk bakteri batang bengkok Gram negatif/batang Gram variabel (Mobilluncus) adalah 49,99% untuk skor 1 dan terkecil adalah 4,8% untuk skor 0. Kesimpulan Hasil penelitian ini menunjukkan bahwa dominasi Lactobacillus sebagai flora normal vagina tampak signifikan mengalami penurunan pada pasien vaginosis bakterial.
