Ekspresi Tumor Necrosis Factor Alpha (TNF-α) pada Penderita Polip Hidung

Abstract

Background: Nasal polyps is an inflammatory process involving the mucosa of the nose and paranasal sinuses. It is a soft mass that contains a lot of fluid in the nasal cavity, grayish-white color, which is caused by inflammation of the mucosa. Nasal polyps about 85-90% by histopathology are eosinophilic polyps characterized by dominant infiltration of eosinophils. Tumor Necrosis Factor Alpha (TNF-α) is a pro-inflammatory cytokine that has an essential role in the pathogenesis of nasal polyps. TNF-α enhance the secretion of VCAM-1 and RANTES in nasal fibroblasts which regulates the eosinophil infiltration, further will accelerate the accumulation of eosinophils in nasal polyps. Purpose: To assess expression of TNF-α in nasal polyps. Methods: This research was a descriptive case series study design, performed on 33 cases of nasal polyps obtained by non-probability consecutive sampling. Immunohistochemistry examination was performed to assess the expression of TNF-α in nasal polyps. Results: From 33 cases of nasal polyps, we found positive expression (overexpression) of TNF-α (84.8%). From all the positive groups, the largest age group is ≥40 years (60,7%), men found more common than women (67,9%), most histopathological type of polyp is type 1 (Allergic polyp) (64,3%) and most polyp stage is stage 3 (50%). Conclusion: Expression of Tumor Necrosis Factor Alpha (TNF-α) is positive (overexpression) in nasal polyp, which is found mostly in the age group ≥40 years, men, allergic type nasal polyps and nasal polyps stage 3.

Latar belakang: Polip hidung adalah proses inflamasi yang melibatkan mukosa hidung dan sinus paranasal. Polip merupakan massa lunak yang mengandung banyak cairan di dalam rongga hidung, berwarna putih keabu-abuan, terjadi akibat inflamasi mukosa. Berdasarkan histopatologi sekitar 85-90% adalah polip eosinofilik ditandai dengan infiltrasi eosinofil yang dominan. Tumor Necrosis Factor (TNF-α) merupakan sitokin pro-inflamasi yang memiliki peranan penting dalam patogenesis polip hidung. TNF-α meningkatkan sekresi dari VCAM-1 dan RANTES pada fibroblas hidung yang meregulasi infiltrasi eosinofil, lebih lanjut akan mempercepat akumulasi eosinofil pada polip hidung. Tujuan: Mengetahui ekspresi TNF-α pada penderita polip hidung. Metode: Penelitian ini bersifat deskriptif dengan desain studi case caries yang dilakukan terhadap 33 kasus polip hidung yang diperoleh secara non probability consecutive sampling. Pemeriksaan imunohistokimia dilakukan untuk menilai ekspresi TNF-α pada polip hidung. Hasil: Dari 33 kasus polip hidung, dijumpai ekspresi positif (overexpression) TNF-α (84,8%). Dari seluruh kelompok positif, usia terbanyak ≥40 (60,7%), laki-laki dijumpai lebih banyak dari perempuan (67,9%), tipe histopatologi polip terbanyak adalah tipe 1 (Allergic polyp) (64,3%), sedangkan stadium polip terbanyak adalah stadium 3 (50%). Kesimpulan : Ekspresi TNF-α dijumpai positif (overexpression) pada polip hidung, di mana dijumpai terbanyak pada laki-laki, kelompok umur ≥40 tahun, polip hidung tipe alergi dan polip stadium 3.
