Faktor Risiko Hipertensi, Dislipidemia, Merokok, Asam Urat, Obesitas, Diabetes Melitus, dan Riwayat Stroke dalam Keluarga pada Penderita Stroke

Abstract

Background : Stroke is a neurological syndrome which is a global health problem, a major cause of disability, and the main cause of mortality worldwide. It is known that there are no special medical measures contribute to the quality of life in old age as the prevention of cerebrovascular disease. Knowing of risk factors needed for the prevention and decrease the number of occurrence of stroke. Purpose : The aim of this study to identify the risk factors for hypertension, dyslipidemia, smoking, gout, obesity, diabetes mellitus, and family history of stroke in the stroke patients. Methods : Study was carried out in Haji Adam Malik General Hospital Medan, North Sumatera. A case control data collection with primary data source was designed to find the risk factors for hypertension, dyslipidemia, smoking, gout, obesity, diabetes mellitus, and family history of stroke. The study consist of 200 subjects with 100 strokes (confirmed with Head Computed Tomography Scan) and 100 non-strokes. Results : The study by using logistic regression analysis and its 95% confidence intervals were indentified, hypertension (OR=6,192), diabetes mellitus (OR=2,346), smoking (OR=1,176), gout (OR=0,736), dyslipidemia (OR=2,124), obesity (OR=1,128), family history of stroke (OR=1,806). Conclusion : This is a study that quantifies the contribution of different factors to the overall risk of stroke. Finding that, hypertension, diabetes mellitus, and dyslipidemia with the significance p<0.05 as the risk factors for all type of strokes and hypertension is the biggest risk factors for all type of strokes.

Latar Belakang : Stroke adalah sindrom neurologis yang merupakan masalah kesehatan global, penyebab utama kecacatan, dan penyebab utama kematian di seluruh dunia. Dimana tidak ada tindakan medis khusus yang berkontribusi terhadap kualitas hidup di usia tua sebagai pencegahan penyakit serebrovaskular. Pengetahuan tentang faktor-faktor risiko diperlukan untuk pencegahan dan menurunkan angka kejadian stroke. Tujuan : Tujuan penelitian ini untuk mengetahui besar risiko hipertensi, dislipidemia, merokok, asam urat, obesitas, diabetes melitus, dan riwayat stroke dalam keluarga terhadap kejadian stroke Metode : Penelitian dilakukan di Rumah Sakit Umum Pusat Haji Adam Malik Medan, Sumatera Utara. Pengumpulan data secara kasus kontrol dengan sumber data primer untuk mengetahui faktor risiko hipertensi, dislipidemia, merokok, asam urat, obesitas, diabetes melitus, dan riwayat stroke dalam keluarga. Penelitian ini terdiri dari 200 subjek dengan stroke 100 orang (dikonfirmasi dengan Computed Tomography Scan kepala) dan bukan stroke 100 orang. Hasil : Penelitian ini dengan menggunakan analisis regresi logistik dan 95% Confidence Interval didapatkan faktor risiko hipertensi (OR=6,192), diabetes melitus (OR=2,346), merokok (OR=1,176), asam urat (OR=0,736), dislipidemia (OR=2,124), obesitas (OR=1,128), riwayat stroke dalam keluarga (OR=1,806). Kesimpulan : Penelitian ini menggabungkan faktor-faktor yang berbeda terhadap faktor risiko stroke secara keseluruhan dengan hasil hipertensi, diabetes melitus, dan dislipidemia sebagai faktor risiko yang bermakna dengan p<0,05 untuk semua jenis stroke dan hipertensi adalah faktor risiko terbesar terhadap terjadinya stroke.
