    Hubungan Kadar Fibrinogen Dengan Hasil Pemeriksaan Transcranial Doppler (TCD) pada Penderita Stroke Iskemik Akut

    Abstract
    Background..Elevated fibrinogen levels is related to the blood hyperviscosity, that causes low blood velocity. Non-contrast-enhanced transcranial Doppler (TCD) is used to evaluate blood flow from the cerebrovascular system. Objective. To evaluate the relationship between fibrinogen levels and examination of TCD in acute ischemic stroke. Methods. A cross-sectional study was admitted from July 2012-Juny 2013. We determined the differences between fibrinogen and TCD in stroke group and control. And we examined the relationship between fibrinogen and TCD examination in stroke group. The fibrinogen was measured with Clauss method, and was carried out at Clinical Pathology Departement, H Adam Malik Hospital and The TCD was examined due to middle cerebry artery (MCA), and internal carotid artery (ICA), and it was carried out at Neurology Departement, H Adam Malik Hospital. The patients were diagnosed as ischemic stroke from head ctscan. Patients with recurrence stroke, pregnancy, liver diseases, inflamation, and who were not examined fibrinogen and/or TCD excluded. Statistical analyses employed the Independent T test, Anova test, and Pearson correlation. Results. We had 24 patients and 24 controls. Fibrinogen levels in stroke group is 549,16±104,84 mg/ml, control group is 385,64±16,80 mg/ml. We examined MCA in stroke group and control, mean velocity 43,12±21,03 dan 56,97±6,22 (p=0.05), peak velocity 74,17±32,58 dan 94,55±14,11 (p=0.05), end diastolic velocity 23,27±12,66 dan 35,30±7,34 (p=0.00). In ICA, stroke group and control, mean velocity 31,40±8,86 dan 43,07±8,06 (p=0.00), peak velocity 54,99±11,50 dan 75,04±16,04 (p=0.00), end diastolic velocity 18,23±7,67 dan 25,64±5,24 (p=0.00). Conclusions. The differences between fibrinogen levels and TCD ( The correlation between fibrinogen and TCD in stroke group was not significant on MCA and ICA, P>0.05. mean velocity, peak velocity, end diastolic velocity) Key words : fibrinogen, transcranial doppler, acute ischemic stroke in stroke group and control are significant. But there is no correlation between fibrinogen and TCD in stroke group.
     
    Latar Belakang. Peningkatan kadar fibrinogen berhubungan dengan viskositas darah, akibatnya terjadi penurunan kecepatan aliran darah. Non kontras Transcranial Doppler (TCD) digunakan untuk mengevaluasi aliran darah dari sistem serebrovaskular. Tujuan. Untuk melihat hubungan antara kadar fibrinogen darah dengan hasil pemeriksaan TCD. Metode. Penelitian dengan metode analitik pendekatan potong lintang, sejak bulan Juli 2012–Juni 2013. Dinilai perbedaan antara kadar fibrinogen dan hasil pemeriksaan TCD pada kelompok stroke dan kontrol. Selanjutnya, dinilai hubungan antara kadar fibrinogen dengan hasil pemeriksaan TCD pada kelompok stroke. Pemeriksaan Fibrinogen dengan metode Clauss dilakukan di Departemen Patologi Klinik RSUP.H.A.Malik .Pemeriksaan TCD dilakukan melalui arteri serebri media (MCA) dan arteri karotis interna (ICA) di Departemen Ilmu Penyakit Syaraf RSUP.H.A.Malik Medan. Diagnosa stroke iskemik ditegakan dengan CT scan kepala, kriteria ekslusi: pasien stroke berulang, kehamilan, penyakit hati, infeksi, dan mereka yang tidak dapat dilakukan kedua pemeriksaan. Perhitungan statistik menggunakan uji T independent, Anova dan korelasi Pearson. Hasil. Didapatkan 24 pasien, dan 25 kontrol. Fibrinogen pada kelompok stroke 549,16±104,84 mg/ml, dan kelompok kontrol 385,64±16,80 mg/ml (p=0.00). Pemeriksaan TCD, di MCA, pada kelompok stroke dan kontrol, didapatkan nilai mean velocity 43,12±21,03 dan 56,97±6,22 (p=0.05), peak velocity 74,17±32,58 dan 94,55±14,11 (p=0.05), end diastolic velocity 23,27±12,66 dan 35,30±7,34 (p=0.00). Di ICA, pada kelompok stroke dan kontrol, yaitu mean velocity 31,40±8,86 dan 43,07±8,06 (p=0.00), peak velocity 54,99±11,50 dan 75,04±16,04 (p=0.00), end diastolic velocity 18,23±7,67 dan 25,64±5,24 (p=0.00). Korelasi fibrinogen dengan pemeriksaan TCD pada kelompok stroke tidak ditemukan hubungan yang signifikan, baik pada MCA, dan ICA, dengan P>0.05. Kesimpulan. Ada perbedaan yang signifikan antara kadar fibrinogen dan hasil pemeriksaan TCD (mean velocity, peak velocity, end diastolic velocity) pada kelompok stroke iskemik akut dan kelompok non stroke. Namun tidak didapati hubungan yang signifikan antara kadar fibrinogen dengan hasil pemeriksaan TCD.
