dc.description.abstractBackground : The Mini-Mental State Exam (MMSE) (Folstein et al., 1975) is probably the most widely used screening measure of cognitive functioning. In the past, the Cognitive Performance Scale (CPS) corresponded closely with scores generated by the MMSE. However, no standard diagnostic accuracy study, comparing both CPS and MMSE with a gold standard, is yet available. Objective : The objective of this study was to compare the diagnostic accuracy of the CPS and the MMSE for the detection of cognitive impairment. The General Practitioner Assessment of Cognition (GPCOG) was used as the reference standard. Methods : This cross sectional study with > 80 subjects more than 60 years old living in UPT Pelayanan Sosial Lanjut Usia dan Anak Balita Wilayah Binjai-Medan and Panti Jompo Karya Kasih Medan. All subjects examined for CPS, MMSE and GPCOG. Results : The diagnostic values of a CPS score of 2 or more for the detection of cognitive impairment were : sensitivity = 0,84; specificity = 0,88; PPV= 0,90; NPV= 0,82, LR + = 7, LR - = 0,18 and accuracy = 0,86. The diagnostic values of a MMSE score of 23 or less for the detection of cognitive impairment were : sensitivity = 0,91; specificity = 0,72; PPV= 0,80; NPV= 0,86, LR + = 3,21, LR - = 0,13 and accuracy = 0,82. Conclusion : There is insignificant difference between CPS and MMSE accuracy to detect cognitive impairment. (p= 0,32)en_US
dc.description.abstractLatar Belakang : Mini Mental State Examination (MMSE) merupakan penilaian fungsi kognitif yang paling luas dipakai. Pada MMSE berbagai domain yang dinilai meliputi orientasi waktu dan tempat, atensi, memori, bahasa dan konstruksi visual. Dahulu skala Cognitive Performance Scale (CPS) dihubungkan secara erat dengan skor MMSE, meskipun tidak ada studi tentang akurasi standar diagnostik yang membandingkan CPS dan MMSE dengan gold standard yang telah digunakan. Tujuan : Untuk mengetahui perbandingan akurasi diagnostik CPS dan MMSE terhadap General Practioner Assessment of Cognition (GPCOG) dalam menilai gangguan fungsi kognitif pada usia lanjut. Metode : Studi cross sectional dengan 80 subjek yang berusia 60 tahun atau lebih yang tinggal di UPT Pelayanan Sosial Lanjut Usia dan Anak Balita Wilayah Binjai-Medan dan Panti Jompo Karya Kasih Medan. Semua subjek dilakukan pemeriksaan CPS, MMSE dan GPCOG. Hasil : Nilai diagnostik untuk nilai CPS ≥ 2 dalam menilai fungsi kognitif usia lanjut didapatkan sensitifitas = 0,84; spesifisitas = 0,88; NDP= 0,90; NDN= 0,82, RKP = 7, RKN = 0,18 dan akurasi = 0,86. Nilai diagnostik untuk nilai MMSE ≤ 23 dalam menilai fungsi kognitif usia lanjut didapatkan sensitifitas = 0,91; spesifisitas = 0,72; NDP = 0,80; NDN= 0,86, RKP = 3,21, RKN = 0,13 and akurasi = 0,82. Kesimpulan : Terdapat perbedaan akurasi yang tidak bermakna antara CPS dan MMSE dalam menilai fungsi kognitif usia lanjut. (p = 0,32)en_US
