    Indeks Trombosit sebagai Indikator Komplikasi Mikrovaskular pada Penderita Diabetes Melitus Tipe 2

    Abstract
    Background : Diabetes mellitus (DM) has been considered as a ‘‘prothrombotic state’ with enhanced platelet reactivity. Platelet activity and aggregation potential, which are essential components of thrombogenesis and atherosclerosis, can be conveniently estimated by measuring mean platelet volume (MPV) as part of whole blood count. The aim of study : This study was aimed to determine platelet indices [MPV, platelet distribution width (PDW) and platelet-large cell ratio (P- LCR)] between Type 2 diabetics with and without microvascular complication. Methods : This study included 70 subjects with Type 2 DM (40 with one or more microvascular complications) and 30 without microvascular complications. Anticoagulated blood (ethylene diamine tetracetic acid) was collected and analyzed in an automated blood cell counter for platelet count and indices. Results: In our study found the MPV, PDW and P-LCR were all significantly higher in subjects without microvascular complication compared to the subjects with microvascular complication [ 9,97±0,68 vs 9,81±0,76 fl ;11,81±1,55 vs 11,44±1,50 fl; 24,93±4,72 vs 23,79±4,88 % (P>0.05 for all) ], respectively. Conclusion: There was no significant differences in platelet indices (MPV, PDW, and P-LCR) between subjects with and without microvascular complication.
     
    Latar belakang : Diabetes melitus (DM) dikenal sebagai ‘‘prothrombotic state’ dengan reaktivitas trombosit yang meningkat. Aktivasi trombosit dan potensi agregasi berperan penting dalam trombogenesis dan aterosklerosis. Dan dapat dengan mudah diperkirakan dengan mengukur mean platelet volume (MPV) yang merupakan bagian dari pemeriksaan darah lengkap. Tujuan Penelitian : Penelitian ini bertujuan untuk menilai indeks trombosit [MPV, platelet distribution width (PDW) and platelet-large cell ratio (P-LCR)] pada penderita DM tipe 2 dengan dan tanpa komplikasi mikrovaskular. Metode penelitian : Subjek penelitian sebanyak 70 orang penderita DM tipe 2 (40 orang dengan 1 atau lebih komplikasi mikrovaskular) and 30 orang tanpa komplikasi mikrovaskular. Darah dengan antikoagulan (ethylene diamine tetracetic acid) diambll dan dianalisa menggunakan automated blood cell counter untuk menilai jumlah dan indeks trombosit. Hasil : Pada penelitian ini dijumpai nilai MPV, PDW dan P-LCR lebih tinggi signifikan pada penderita DM tipe 2 tanpa komplikasi mikrovaskular dibandingkan dengan subjek yang dengan komplikasi mikrovaskular [ 9,97±0,68 vs 9,81±0,76 fl ;11,81±1,55 vs 11,44±1,50 fl; 24,93±4,72 vs 23,79±4,88 % (P>0.05) ], secara berurutan. Kesimpulan : Tidak dijumpai perbedaan bermakna pada nilai indeks trombosit (MPV, PDW, dan P-LCR) antara penderita DM tipe 2 dengan dan tanpa komplikasi mikrovaskular.
