    Kaitan Glasgow Coma Score Awal dan Jarak Waktu Setelah Cedera Kepala Sampai Dilakukan Operasi pada Pasien Perdarahan Subdural Akut dengan Glasgow Outcome Scale

    Abstract
    Objective: Acute subdural haemorrhage is one of the neurosurgical diseases that have: The mortality is relatively high whether the patient is operated on or not. Therefore, subdural hemorrhage need attention both in knowledge of pathophysiology and in mastery action to deal with it. Aykut Karasu et al in Istanbul Turkey said that the prognostic factors of paramount importance in patients with acute SDH who underwent surgery was the GCS at initial admission, whereas Seeling et al in Germany said that acute SDH patients who underwent surgery within < 4 hours has a mortality of 30% and if more than 4 hours has a mortality of 90%. Materials and Methods of Operation: All patients with acute SDH who underwent surgery were recorded for GCS on the at the time of initial admission and the time interval from head injury to surgery, then assessed Glasgow postoperative outcome scale at the time the patient was discharged. Results: From 23 cases of acute SDH who were operated on, statistically the association between GCS at initial admission where the GOS value is weak and not significant (p = 0.06; r = 0.41) and the relationship between the distance after head injury until surgery with GOS value is very strong and significant (p = 0.001; r = (-)0.66) , where the sooner the patient with acute subdural hemorrhage is treated surgery, the better the prognosis. Conclusion: The time interval after head injury until surgery is estimated can be used as a predictor of prognosis in patients with acute subdural hemorrhage.
     
    Objektif : Perdarahan subdural akut merupakan salah satu penyakit bedah syaraf yang mempunyai mortalitas relative tinggi apakah penderita dioperasi atau tidak. Oleh karena itu perdarahan subdural perlu mendapat perhatian baik di dalam pengetahuan patofisiologinya maupun di dalam penguasaan tindakan menanggulanginya. Aykut karasu dkk di Istambul Turki mengatakan bahwa faktor prognostik terpenting terpenting pada pasien SDH akut yang di operasi adalah GCS pada awal masuk, sedangkan Seeling dkk di Jerman mengatakan bahwa pasien SDH akut yang dilakukan operasi dalam waktu < 4 jam mempunyai mortalitas 30 % dan jika lebih > 4 jam mempunyai mortalitas 90 %. Bahan dan Cara kerja : Seluruh pasien SDH akut yang dilakukan operasi dilakukan pencatatan GCS pada saat awal masuk dan jarak waktu cedera kepala sampai dilakukan operasi, kemudian dinilai Glasgow outcome scale pasca Operasi pada saat pasien dipulangkan. Hasil : Dari 23 kasus SDH akut yang dioperasi, secara statistik hubungan antara GCS saat awal masuk dengan nilai GOS adalah lemah dan tidak bermakna (p = 0,06; r = 0,41) dan hubungan antara jarak waktu setelah cedera kepala sampai dilakukan operasi dengan nilai GOS adalah sangat kuat dan signifikan (p = 0,001; r = (-)0,66) , dimana semakin cepat penderita perdarahan subdural akut dilakukan tindakan operasi, maka semakin baik prognosisnya. Simpulan : Jarak waktu setelah cedera kepala sampai dilakukan operasi diperkirakan dapat digunakan sebagai prediktor prognosis penderita perdarahan subdural akut.
